目的：总结特发性肺间质纤维化（IPF）的影像表现及动态变化，重点关注蜂窝影与磨玻璃密度影的表现及变化，并与病理结果对照。方法回顾性分析41例特发性肺间质纤维化（IPF）的HRCT表现，总结其各种征象的出现率和病变的分布情况，并与其中3例行病理检查的进行影像-病理对照观察。结果41例IPF中可见蜂窝影33例（80.5％），磨玻璃密度影34例（82．9％），牵拉性支扩35例（85．3％），网状影37例（90．2％）小叶间隔增厚33例（80.5％）。动态观察中蜂窝影10例（100％）随访中范围增大，磨玻璃密度影中9例（52．9％，9/17）范围变小，相应区域出现蜂窝影。3例患者行肺叶切除病理表现符合普通型间质性肺炎，其中1例胸膜下区可见细支气管扩张。结论磨玻璃密度和网状影、小叶间隔增厚是纤维化的早期表现。其病理基础是肺泡间隔纤维增生，小叶间隔及小叶内间质增生。
목적：총결특발성폐간질섬유화（IPF）적영상표현급동태변화，중점관주봉와영여마파리밀도영적표현급변화，병여병리결과대조。방법회고성분석41례특발성폐간질섬유화（IPF）적HRCT표현，총결기각충정상적출현솔화병변적분포정황，병여기중3례행병리검사적진행영상-병리대조관찰。결과41례IPF중가견봉와영33례（80.5％），마파리밀도영34례（82．9％），견랍성지확35례（85．3％），망상영37례（90．2％）소협간격증후33례（80.5％）。동태관찰중봉와영10례（100％）수방중범위증대，마파리밀도영중9례（52．9％，9/17）범위변소，상응구역출현봉와영。3례환자행폐협절제병리표현부합보통형간질성폐염，기중1례흉막하구가견세지기관확장。결론마파리밀도화망상영、소협간격증후시섬유화적조기표현。기병리기출시폐포간격섬유증생，소협간격급소협내간질증생。
Objective To generalize the HRCT menifestions and dynamic change of idiopathic pulmonary fibrosis (IPF) and to correlate them to pathologic findings .Methods We analyzed 41 cases of IPF ,which were proved by cilinic and pathlogy .Results In 41 cases of IPF honeycoming ,ground grass opacity (GGO) ,traction brochilectisis ,reticulatic opac-ity ,interlobular thickening were seen in 33 cases (80 .5% ) ,34 cases (82 .9% ) ,35cases (85 .3% ) ,37 cases (90 .2% ) and 33 cases (80.5% ) ,respectively .In 17 cases with follow-up CT ,the extent of honecombing in 10 cases was found to be more than intial CT ,in which honeycombing cyst was enlarged in 1 case .GGO in 9 of 17 cases was found to develop to ho-neycombing .Lobectomy was performed in 3 cases due to lung cancer .Usual interstitial pneumonia was diagnosed in the three cases .Conclusion GGO ,reticulatic opacity and interlobular thickening were early stage CT findings of IPF .GGO and reticulatic opacity were consistent with alveolar septal thickening ,interlobular and intralobular fibroblast proliferation .
