目的:探讨原发性中枢神经系统淋巴瘤的影像学表现,以提高影像诊断水平。方法回顾性分析25例经病理证实的原发性中枢神经系统淋巴瘤的C T、M RI等影像学资料。结果单发16例,多发9例,共有39个病灶,幕上35个病灶,幕下4个病灶。单发病灶多位于脑白质深部,多发病灶常位于脑实质中线部位及大脑凸面。肿瘤在CT表现呈稍高密度,病灶T1 WI多呈等或稍低信号,T2 WI多呈等或稍高信号,DWI多呈高信号。增强扫描多呈明显均匀强化,少数呈斑片状强化;瘤周可见不同程度的水肿。结论原发性中枢神经系统淋巴瘤的C T、M RI表现具有一定特征性,结合临床并综合其影像学表现,可提高其诊断准确率。
目的:探討原髮性中樞神經繫統淋巴瘤的影像學錶現,以提高影像診斷水平。方法迴顧性分析25例經病理證實的原髮性中樞神經繫統淋巴瘤的C T、M RI等影像學資料。結果單髮16例,多髮9例,共有39箇病竈,幕上35箇病竈,幕下4箇病竈。單髮病竈多位于腦白質深部,多髮病竈常位于腦實質中線部位及大腦凸麵。腫瘤在CT錶現呈稍高密度,病竈T1 WI多呈等或稍低信號,T2 WI多呈等或稍高信號,DWI多呈高信號。增彊掃描多呈明顯均勻彊化,少數呈斑片狀彊化;瘤週可見不同程度的水腫。結論原髮性中樞神經繫統淋巴瘤的C T、M RI錶現具有一定特徵性,結閤臨床併綜閤其影像學錶現,可提高其診斷準確率。
목적:탐토원발성중추신경계통림파류적영상학표현,이제고영상진단수평。방법회고성분석25례경병리증실적원발성중추신경계통림파류적C T、M RI등영상학자료。결과단발16례,다발9례,공유39개병조,막상35개병조,막하4개병조。단발병조다위우뇌백질심부,다발병조상위우뇌실질중선부위급대뇌철면。종류재CT표현정초고밀도,병조T1 WI다정등혹초저신호,T2 WI다정등혹초고신호,DWI다정고신호。증강소묘다정명현균균강화,소수정반편상강화;류주가견불동정도적수종。결론원발성중추신경계통림파류적C T、M RI표현구유일정특정성,결합림상병종합기영상학표현,가제고기진단준학솔。
Objective To explore the imaging features of primary lymphoma in central nervous system so as to improve the diagnostic accuracy of imaging .Methods A retrospective analysis of imaging findings was performed on 25 cases with pathologically confirmed central nervous system lymphoma .Features of CT or MRI appearances of the tumors were stud-ied .Results On CT ,MRI imaging ,single lesions were found in 16 cases and multiple ones in 9 .The single lesions were commonly located in deep white matter ,while these multiple lesions were more commonly located in midline structure and convex regions of brain hemisphere .These lesions were isointensity or slightly hyperintenity on CT images ,and isointense or slightly hypo-intense on T1 weighted images ,and isointense or slightly hyper-intense on T2 weighted images .The le-sions were hyperintensity on diffusion weighted image .In contrast enhancement ,those lesions displayed in a intense and homogeneous pattern .Different edema existed around these lesions .Conclusion The imaging findings of primary lympho-ma in central nervous system have some characteristics ,its diagnostic accurate rate can be improved by considering both the CT and MRI features and the patient’s clinical information .
