目的分析在基层医院开放性颅脑损伤的延续性治疗.方法 78例开放性颅脑损伤患者中,45例行开颅手术,将开放伤转为闭合伤；单纯清创缝合头皮裂口,颅内开放伤未处理者33例,所有病例均采用抗生素规则治疗1周以上.结果 5例GCS评分在6分以下的病例死亡,66例治愈出院,8例未愈,自动出院.7例行开颅去大骨瓣减压者,其中5例于3～6个月返院行颅骨修补术均恢复良好.结论开放性颅脑损伤患者,均需要手术室及时行清创手术,将开放性颅脑损伤转为闭合性损伤.但在基层医院对开放程度相对较轻,又因各种原因不能行手术的病人,严格行单纯清创缝合术,术后采用抗生素规则治疗1周以上,同样可以达到很好的治疗效果.

Objective To analyze continuous treatment for open craniocerebral injury in primary hospitals.Methods Of 78 patients with open craniocerebral injury,45 underwent craniotomy.The open wound was changed to closed injury.The scalp wounds were debrided and sutured.33 patients received no treatment for open cerebral trauma.All the patients were adminstered with antibiotics for more than one week.Results 6 patients with a GCS score below 5 were dead; 66 were cured; and 8 were not cured and left from hospital automatically.Of 7 patients undergoing decompressive craniotomy with large bone flap removal,5 received repair of skull bone and recovered well.Conclusions Patients with open craniocerebral injury need timely debridement surgery,and open craniocerebral injury should be changed to closed injury.For the patients with mild open injury and unsuitable for surgical treatment because of various causes,we should perform single debridement and give regular postoperative antibiotic therapy for more than one week.
