目的探讨肺原发性霍奇金淋巴瘤的诊断与鉴别诊断.方法回顾性分析3例肺霍奇金淋巴瘤病例并复习相关文献.结果 3例患者均为男性,以咳嗽症状为主,肿瘤较大,平均直径为5 cm,可伴有肺门及纵隔淋巴结肿大,镜下均可见典型的RS细胞,背景细胞内有较明显嗜酸粒细胞浸润,并见干尸细胞,免疫组化CD15和CD30阳性.化疗、骨髓移植可使病情缓解,长期生存.结论肺原发性霍奇金淋巴瘤病理学及免疫表型具有一定的特征,恰当的治疗预后良好.

Objective To explore the diagnosis and differential diagnosis standards of primary pulmonary Hodgkin's Iymphoma (PPHL).Methods Three cases of PPHL were reported and the related literatures were reviewed.Results Three cases were all male with cough.The average diameter of the tumor was 5 cm,with or without hilar and mediastinal lymphadenectasis.Typical R-S cells could be detected through fluorescence microscope in all three cases concomitant moderate eosinophils infiltration,mummy cells and CD15 and CD30-in immunohistochemistry detection.The chemotherapy and bone marrow transplantation could release the symptoms and prolong the survival period.Conclusions There are certain pathological and immunophenotype characteristics in PPHL.The prognosis is optimistic with suitable treatment.
