目的：探讨后路经关节螺钉内固定治疗创伤性寰枢椎不稳定疗效。方法采用以枢椎下关节突下缘中点为进钉点的后路改良节螺钉内固定治疗寰枢椎不稳定46例。结果术后随访5～48个月，平均21个月。寰枢椎获解剖复位44例，未解剖复位2例，内固定位置均良好。寰枢椎于术后2～3个月均获得骨性融合，临床症状缓解，无并发症发生。结论后路改良经关节螺钉内固定术，操作简便，疗效可靠，可作为治疗寰枢椎不稳定的有效术式。
목적：탐토후로경관절라정내고정치료창상성환추추불은정료효。방법채용이추추하관절돌하연중점위진정점적후로개량절라정내고정치료환추추불은정46례。결과술후수방5～48개월，평균21개월。환추추획해부복위44례，미해부복위2례，내고정위치균량호。환추추우술후2～3개월균획득골성융합，림상증상완해，무병발증발생。결론후로개량경관절라정내고정술，조작간편，료효가고，가작위치료환추추불은정적유효술식。
Objective To explore the effects on posterior transarticular screw fixation for traumat-ic atlantoaxial instability.Methods A total of 46 cases of atlantoaxial instability underwent modified pos-terior transarticular screw fixation through the central point of lower margin of the axial inferior articular process.Results All cases were followed up for 5 to 48 months(21 months on average).Anatomical res-toration was achieved in 44 out of the 46 cases,and the position of internal fixation was satisfactory.Atlan-toaxial bony fusion was obtained within two to three months.The clinical symptoms were relieved and no complications occurred.Conclusion Modified posterior transarticular screw fixation is recommended as an effective treatment for atlantoaxial instability due to its simple procedure and satisfying curative effect.
