目的:比较陶对陶(ceramic-on-ceramic,COC)与陶对聚乙烯假体(ceramic-on-polyeth-ylene,COP)在全髋关节置换术中应用的可靠性和耐用性。方法对77例接受初次全髋关节置换的患者进行回顾性研究,比较 COC 与 COP 两种不同假体术后髋关节功能及并发症。结果COC组与 COP 组术后髋关节 Harris 评分分别为92.6±5.1和91.3±4.2,差异无统计学意义(P >0.05);两者在翻修、脱位、陶瓷破碎、异响、松动、骨溶解、深部感染、异化骨化发生率的比较,差异无统计学意义(P >0.05)。结论COC 与 COP 假体均能获得较好的临床疗效,两者术后在髋关节功能及并发症方面疗效相当。

목적:비교도대도(ceramic-on-ceramic,COC)여도대취을희가체(ceramic-on-polyeth-ylene,COP)재전관관절치환술중응용적가고성화내용성。방법대77례접수초차전관관절치환적환자진행회고성연구,비교 COC 여 COP 량충불동가체술후관관절공능급병발증。결과COC조여 COP 조술후관관절 Harris 평분분별위92.6±5.1화91.3±4.2,차이무통계학의의(P >0.05);량자재번수、탈위、도자파쇄、이향、송동、골용해、심부감염、이화골화발생솔적비교,차이무통계학의의(P >0.05)。결론COC 여 COP 가체균능획득교호적림상료효,량자술후재관관절공능급병발증방면료효상당。
Objective To compare the reliability and durability of ceramic-on-ceramic(COC)and ceramic-on-polyethylene(COP)bearing surfaces in total hip arthroplasty(THA).Methods A retrospec-tive study was performed on 77 patients who received primary THA.Hip joint functions and complications between the two groups were observed for comparison.Results Postoperative Hip Harris scores(HHS) were(92.6 ±5.1)and(91.3 ±4.2)in the COC group and COP group,respectively.The difference was not significant(P >0.05 ).There were no significant differences in revision,dislocation,ceramic fracture, squeaking,loosening,osteolysis,deep infection and heterotopic ossification between the two groups(P >0.05).Conclusion Both of COC and COP bearing surfaces can get excellent therapeutic effects in THA. There are similar results on postoperative hip function and complications between the two kinds of prosthe-ses.
