目的:探讨磁共振成像技术在原发性三叉神经痛微血管减压术前诊断中的应用价值。方法38例原发性三叉神经痛患者经过三维时间飞跃法磁共振血管成像序列(3D-TOF-MRA)检查。结果38例患者中术前序列图像上显示血管神经压迫症状侧阳性率89.5%(34/38),非症状侧阳性率15.8%(6/38),两侧阳性率差异有统计学意义(P <0.05);手术中见周围血管压迫神经者36例(94.7%),未压迫神经者2例(5.3%),3D-TOF-MRA 诊断符合率97.1%(33/34),术后症状消失或缓解37例(97.4%)。结论3D-TOF-MRA 可清楚地显示三叉神经与责任血管的关系,在原发性三叉神经痛微血管减压术术前诊断及术中显微定位中具有较高的临床应用价值。
目的:探討磁共振成像技術在原髮性三扠神經痛微血管減壓術前診斷中的應用價值。方法38例原髮性三扠神經痛患者經過三維時間飛躍法磁共振血管成像序列(3D-TOF-MRA)檢查。結果38例患者中術前序列圖像上顯示血管神經壓迫癥狀側暘性率89.5%(34/38),非癥狀側暘性率15.8%(6/38),兩側暘性率差異有統計學意義(P <0.05);手術中見週圍血管壓迫神經者36例(94.7%),未壓迫神經者2例(5.3%),3D-TOF-MRA 診斷符閤率97.1%(33/34),術後癥狀消失或緩解37例(97.4%)。結論3D-TOF-MRA 可清楚地顯示三扠神經與責任血管的關繫,在原髮性三扠神經痛微血管減壓術術前診斷及術中顯微定位中具有較高的臨床應用價值。
목적:탐토자공진성상기술재원발성삼차신경통미혈관감압술전진단중적응용개치。방법38례원발성삼차신경통환자경과삼유시간비약법자공진혈관성상서렬(3D-TOF-MRA)검사。결과38례환자중술전서렬도상상현시혈관신경압박증상측양성솔89.5%(34/38),비증상측양성솔15.8%(6/38),량측양성솔차이유통계학의의(P <0.05);수술중견주위혈관압박신경자36례(94.7%),미압박신경자2례(5.3%),3D-TOF-MRA 진단부합솔97.1%(33/34),술후증상소실혹완해37례(97.4%)。결론3D-TOF-MRA 가청초지현시삼차신경여책임혈관적관계,재원발성삼차신경통미혈관감압술술전진단급술중현미정위중구유교고적림상응용개치。
Objective To discuss the application value of three-dimensional time of flight mag-netic resonance angiography(3D-TOF-MRA)in microvascular decompression(MVD)for idiopathic trigemi-nal neuralgia(ITN).Methods Thirty-eight patients with ITN received 3D-TOF-MRA.Result Preopera-tive 3D-TOF-MRA sequence images showed that the positive rate of the side of symptomatic neurovascular compression was 89.5%(34 /38)and the positive rate of non-symptomatic side was 15.8%(6 /38).The difference in positive rates between the two sides was significant(P <0.05).During the operation,there were 36 cases(94.7%)with neurovascular compression and 2 cases(5.3%)without it.The diagnostic ac-cordance rate of 3D-TOF-MRA was 97.1%(33 /34).After the operation,the symptoms were disappeared or relieved in 37 cases(97.4%).Conclusion 3D-TOF-MRA can clearly reveal the relationship between the trigeminal nerve and responsible vessels,which is of high clinical value for the preoperative diagnosis and micro-positioning in MVD for ITN.
