目的:回顾分析室间质评1304号菌株的鉴定过程,评价 Vitek 2 Compact 全自动微生物分析系统(简称 Vitek 2 Compact 系统)在临床少见疑难细菌鉴定中的应用。方法采用传统的细菌形态学检查、手工生化反应及 Vitek 2 Compact 系统等多种手段对1304号菌株进行鉴定分析,同时用基质辅助激光解吸-电离飞行时间质谱(MALDI-TOF MS)仪对最终鉴定结果进行佐证。结果1304号菌株为革兰阴性小杆菌,氧化酶试验阳性,初次上机鉴定结果为支气管炎鲍特菌,鉴定率99%,转种分纯后2次上机鉴定结果为低分辨,粪产碱杆菌占51%,类产碱假单胞菌占49%,补充硝酸盐还原试验,结果为阴性,报告最终鉴定结果为粪产碱杆菌,质谱鉴定得分值为2.158,与最终结果相符。结论Vitek 2 Compact 系统鉴定临床少见疑难细菌有一定局限性,遇到少见疑难细菌应该用多种方法加以确定,综合分析以免造成错误鉴定。
目的:迴顧分析室間質評1304號菌株的鑒定過程,評價 Vitek 2 Compact 全自動微生物分析繫統(簡稱 Vitek 2 Compact 繫統)在臨床少見疑難細菌鑒定中的應用。方法採用傳統的細菌形態學檢查、手工生化反應及 Vitek 2 Compact 繫統等多種手段對1304號菌株進行鑒定分析,同時用基質輔助激光解吸-電離飛行時間質譜(MALDI-TOF MS)儀對最終鑒定結果進行佐證。結果1304號菌株為革蘭陰性小桿菌,氧化酶試驗暘性,初次上機鑒定結果為支氣管炎鮑特菌,鑒定率99%,轉種分純後2次上機鑒定結果為低分辨,糞產堿桿菌佔51%,類產堿假單胞菌佔49%,補充硝痠鹽還原試驗,結果為陰性,報告最終鑒定結果為糞產堿桿菌,質譜鑒定得分值為2.158,與最終結果相符。結論Vitek 2 Compact 繫統鑒定臨床少見疑難細菌有一定跼限性,遇到少見疑難細菌應該用多種方法加以確定,綜閤分析以免造成錯誤鑒定。
목적:회고분석실간질평1304호균주적감정과정,평개 Vitek 2 Compact 전자동미생물분석계통(간칭 Vitek 2 Compact 계통)재림상소견의난세균감정중적응용。방법채용전통적세균형태학검사、수공생화반응급 Vitek 2 Compact 계통등다충수단대1304호균주진행감정분석,동시용기질보조격광해흡-전리비행시간질보(MALDI-TOF MS)의대최종감정결과진행좌증。결과1304호균주위혁란음성소간균,양화매시험양성,초차상궤감정결과위지기관염포특균,감정솔99%,전충분순후2차상궤감정결과위저분변,분산감간균점51%,류산감가단포균점49%,보충초산염환원시험,결과위음성,보고최종감정결과위분산감간균,질보감정득분치위2.158,여최종결과상부。결론Vitek 2 Compact 계통감정림상소견의난세균유일정국한성,우도소견의난세균응해용다충방법가이학정,종합분석이면조성착오감정。
Objective To retrospectively analyze the identification process of strain 1 304 in external quality assessment,and to evaluate the application of Vitek 2 Compact automatic microorganism analyzer system (Vitek 2 Compact system)to identify clinical rare bacteria.Methods Strain 1 304 was identified comprehensively by traditional microscopic morphology,manual biochemical reaction and Vitek 2 Compact system,simultaneously.The final result was confirmed by matrix-assisted laser desorption/ionization time-of-flight mass spectrometry(MALDI-TOF MS).Results Strain 1 304 was Gram-negative bacilli.The result of oxidase test was positive.The first result was Bordetella bronchiseptica (identification probability was 99%).The second result was low discrimination(51 % Alcaligenes faecalis and 49% Pseudomonas pseudoalcaligenes)by Vitek 2 Compact system.The result of supplementary test(NO3 )was negative.The final identification result was Alcaligenes faecalis,and MALDI-TOF MS got the same result with a score value of 2.1 58.Conclusions It exists some limitations in the identification of rare bacteria by Vitek 2 Compact system.Comprehend means should be used to decrease wrong identification rate.
