目的:通过使用气囊导管扩张血管的方法,提高血管条件不良的血透患者建立动静脉内瘘的成功率及使用率。方法:总结分析23例血管纤细患者,分别采用常规手术方式及经气囊导管扩张后行动静脉内瘘术患者的资料。结果:常规手术方式建立的动静脉内瘘平均成熟时间大于4周,一年内内瘘失功率达40%,而经气囊导管扩张后的内瘘成熟时间4周,一年内未发生内瘘失功。结论:终末期肾脏病需要建立血管通路完成维持生命的血液透析,自体动静脉内瘘(AVF)是血液透析患者血管通路的首选,在血管条件不理想时,使用气囊导管扩张血管,仔细吻合血管,能够建立良好的血管通路。

목적:통과사용기낭도관확장혈관적방법,제고혈관조건불량적혈투환자건립동정맥내루적성공솔급사용솔。방법:총결분석23례혈관섬세환자,분별채용상규수술방식급경기낭도관확장후행동정맥내루술환자적자료。결과:상규수술방식건립적동정맥내루평균성숙시간대우4주,일년내내루실공솔체40%,이경기낭도관확장후적내루성숙시간4주,일년내미발생내루실공。결론:종말기신장병수요건립혈관통로완성유지생명적혈액투석,자체동정맥내루(AVF)시혈액투석환자혈관통로적수선,재혈관조건불이상시,사용기낭도관확장혈관,자세문합혈관,능구건립량호적혈관통로。
Objective:To improve the success rate and utilization rate of internal arteriovenous fistula operation for hemodialysis with balloon catheter to dilate vessel under the circumstances of ad blood vessel.Methods:The clinical data of 23 patients with slender blood vessel undergoing internal arteriovenous fistula operation with conventional operation mode and the balloon catheter dilating vessel were retrospectively analyzed.Results:The average maturity time of internal arteriovenous fistula with conventional operation mode was more than 4 weeks,the losing power of internal fistula was 40% within a year,the av-erage maturity time of internal arteriovenous fistula with balloon catheter was 4 weeks,and there was no fistula dysfunction.Conclusion:Hemodialysis ESRD needs to establish a vascular access to complete a life sustaining,autogenous arteriovenous fistula (AVF)is a vascular access for hemodialysis patients pre-ferred,under the circumstances of bad blood vessel,balloon catheter to dilate vessel is an effective procedure to keep vascular patency in patiency in patients undergoing internal arteriovenous fistula operation hemodialysis.
