目的：分析前列腺癌根治术后 tPSA 异常的影响因素，为前列腺癌患者个体化后续治疗提供依据。方法回顾性分析2004年1月至2013年10月在广西医科大学第一附属医院及附属肿瘤医院行前列腺癌根治术的72例前列腺癌患者的临床资料。单因素分析研究各参数对根治术后1个月 tPSA 异常的影响，多因素 Logistic 回归分析影响根治术后 tPSA 异常的独立因素。结果单因素分析显示年龄、前列腺体积、直肠指检、影像学表现、术前 Gleason 评分及临床分期对评估术后 tPSA 值异常无统计学意义(P >0.05)。新辅助治疗、术前 tPSA 水平、病理分期、术后 Gl-eason 评分、淋巴结转移及切缘情况对评估术后 tPSA 值异常有统计学意义(P <0.05)。多因素分析显示术后 Gleason 评分、淋巴结转移及切缘情况是术后 tPSA 异常的独立影响因素。结论术后 Gleason 评分、淋巴结转移、切缘情况是影响前列腺根治术后患者 tPSA 异常的独立因素。
목적：분석전렬선암근치술후 tPSA 이상적영향인소，위전렬선암환자개체화후속치료제공의거。방법회고성분석2004년1월지2013년10월재엄서의과대학제일부속의원급부속종류의원행전렬선암근치술적72례전렬선암환자적림상자료。단인소분석연구각삼수대근치술후1개월 tPSA 이상적영향，다인소 Logistic 회귀분석영향근치술후 tPSA 이상적독립인소。결과단인소분석현시년령、전렬선체적、직장지검、영상학표현、술전 Gleason 평분급림상분기대평고술후 tPSA 치이상무통계학의의(P >0.05)。신보조치료、술전 tPSA 수평、병리분기、술후 Gl-eason 평분、림파결전이급절연정황대평고술후 tPSA 치이상유통계학의의(P <0.05)。다인소분석현시술후 Gleason 평분、림파결전이급절연정황시술후 tPSA 이상적독립영향인소。결론술후 Gleason 평분、림파결전이、절연정황시영향전렬선근치술후환자 tPSA 이상적독립인소。
Objective To investigate the influencing factors of abnormal tPSA after radical prostatectomy for providing the basis for individualized post-operation therapy of patients. Methods The clinical and pathological data of 72 cases who accepted radical prostatectomy were retrospec-tively analyzed in the First Affiliated Hospital and Affiliated Tumor Hospital of Guangxi Medical U-niversity between January 2004 and October 2013.Univariate analysis was used to estimate the rela-tionship between the parameters and abnormal tPSA (one month after operation testing).Multivari-able logistic regression analysis were used to determine influencing factors for abnormal tPSA. Re-sults The univariate analysis showed that patient's age,prostate volume,digital rectal examina-tion,imaging findings,preoperative Gleason score and preoperative clinical stage had negative prog-nostic significance (P >0.05 ).Neoadjuvant treatment,tPSA level,pathological stage,postopera-tive Gleason score,lymph node metastasis and margin status differences were positive prognostic significant (P <0.05 ).The multifactor analysis revealed that postoperative Gleason score,lymph node metastasis states and margin status were independent predictors of abnormal tPSA. Conclu-sions Postoperative Gleason score,lymph node metastasis,margin status were independent influen-cing factors of abnormal tPSA after radical prostatectomy.
