目的：分析评价普外科Ⅰ类切口手术抗菌药物的应用情况，为临床合理用药提供依据。方法选取243例普外科Ⅰ类切口手术患者，其中2011年123例，2012年120例，对比分析2年的预防性用药情况、抗菌药物初次给药时间及疗程分布、抗菌药物的联合应用情况以及抗菌药物种类及用药频度分布情况。结果2011年预防性用药明显多于2012年，差异有统计学意义（P＜0.05）。2011、2012年预防性用药患者抗菌药初次给药及疗程分布的差异均无统计学意义（P＞0.05）。2011年预防性用药的54例患者中单一用药39例（72.22%），二联用药15例（27.78%），共用抗菌药物5类7种；2012年27例预防性用药均为单一用药，共用抗菌药物3类4种。结论2012年Ⅰ类切口手术抗菌药物应用较之2011年更趋合理，使用率明显下降，抗菌药物使用管理成效显著。但初次给药的时机和使用时间还需进一步规范。
목적：분석평개보외과Ⅰ류절구수술항균약물적응용정황，위림상합리용약제공의거。방법선취243례보외과Ⅰ류절구수술환자，기중2011년123례，2012년120례，대비분석2년적예방성용약정황、항균약물초차급약시간급료정분포、항균약물적연합응용정황이급항균약물충류급용약빈도분포정황。결과2011년예방성용약명현다우2012년，차이유통계학의의（P＜0.05）。2011、2012년예방성용약환자항균약초차급약급료정분포적차이균무통계학의의（P＞0.05）。2011년예방성용약적54례환자중단일용약39례（72.22%），이련용약15례（27.78%），공용항균약물5류7충；2012년27례예방성용약균위단일용약，공용항균약물3류4충。결론2012년Ⅰ류절구수술항균약물응용교지2011년경추합리，사용솔명현하강，항균약물사용관리성효현저。단초차급약적시궤화사용시간환수진일보규범。
Objective Analysis and evaluation of the application of antibacterial drugs in type I incision operation in De-partment of General Surgery, which provides a basis for clinical rational drug use. Methods The application of antibacterial drugs in 243 surgical incision patients with class I in our hospital from January, 2011 to December, 2012 were retrospectively analyzed. Results To discover in 123 patients in 2011, 54 cases are preventive medication use, which accounted for 43.90%. In 120 patients in 2012, 27 cases are preventive medication use, which accounted for 22.5%. After statistics processing,x2=12. 52, P<0.05. The first time of administration and duration are of no obvious difference (x2=4.24 and 3.53, P>0.05). As for the combination of antibacterial drugs in 2011, the single drug use accounted for 72.22%,and the bigeminy of the drug use ac-counted for 27.78%. While in 2012, al the drug use is single. Conclusion The application of antibacterial drugs in type I inci-sion operation in 2012 is more rational than that in 2011. And the use rate was obviously decreased. The management of appli-cation of antibacterial drugs made remarkable results. But the first time of administration and duration need to be further stan-dardized.
