目的:探讨高分子量细胞角蛋白34βE12、肌上皮标记物p63、细胞角蛋白CK5/6、平滑肌肌动蛋白α-SMA在前列腺基底细胞中的表达及意义。方法采用免疫组织化学SP法检测,观察34βE12、p63、CK5/6、α-SMA在前列腺增生(PH)、低级别上皮内瘤变(LGPIN)、高级别上皮内瘤变(HGPIN)、前列腺癌(Pca)中的表达。结果30例PH组标本34βE12、p63、CK5/6大多阳性表达,10例LGPIN组多(++)表达,15例HGPIN组多(+)表达, Pca组大多阴性表达。Pca组与PH组、LGPIN组、HGPIN组比较差异有统计学意义(P<0.05)。α-SMA基底细胞少量表达,Pca组与PH组、LGPIN组、HGPIN组比较差异均无统计学意义(P>0.05)。结论前列腺基底细胞表达34βE12、p65、CK5/6,p65优于34βE12,α-SMA多阴性表达。在前列腺标本中联合检测34βE12、p65、CK5/6有助于对前列腺癌及其他不同病变的鉴别诊断。

목적:탐토고분자량세포각단백34βE12、기상피표기물p63、세포각단백CK5/6、평활기기동단백α-SMA재전렬선기저세포중적표체급의의。방법채용면역조직화학SP법검측,관찰34βE12、p63、CK5/6、α-SMA재전렬선증생(PH)、저급별상피내류변(LGPIN)、고급별상피내류변(HGPIN)、전렬선암(Pca)중적표체。결과30례PH조표본34βE12、p63、CK5/6대다양성표체,10례LGPIN조다(++)표체,15례HGPIN조다(+)표체, Pca조대다음성표체。Pca조여PH조、LGPIN조、HGPIN조비교차이유통계학의의(P<0.05)。α-SMA기저세포소량표체,Pca조여PH조、LGPIN조、HGPIN조비교차이균무통계학의의(P>0.05)。결론전렬선기저세포표체34βE12、p65、CK5/6,p65우우34βE12,α-SMA다음성표체。재전렬선표본중연합검측34βE12、p65、CK5/6유조우대전렬선암급기타불동병변적감별진단。
Objective To explore the expression and significance of high molecular weight cytokeratin (34βE12), myoepithelium marker (p65), cytokeratin (CK5/6) and smooth muscle actin (α-SMA ) in the basic cells of pros-tate. Methods Immunohistochemical SP method was performed to detect the expression of 34βE12, p65, CK5/6 andα-SMA in prostatic hyperplasia (PH), low-grade intraepithelial neoplasia (LGPIN), high-grade intraepithelial neopla-sia (HGPIN) and prostate cancer (Pca).Results In 30 PH samples, 34βE12, p65 and CK5/6 were expressed (++) in 10 LGPIN samples, (+) in 15 HGPIN samples and negative in Pca samples. There were significant statistical differenc-es between Pca group and PH, LGPIN and HGPIN groups. Littleα-SMA was expressed in basal cells and there was no significant statistical difference between these groups (P>0.05). Conclusion 34βE12, p65 and CK5/6 were ex-pressed in prostate basic cells and p65 was expressed more than 34βE12.α-SMA was negative in most of the prostatic basic cells. Joint detection of 34βE12, p65 and CK5/6 was helpful for the differential diagnosis of prostate cancer.
