目的:探讨纳洛酮用于新生儿重度窒息治疗时对患儿神经功能及大脑血供的影响。方法86例重度窒息新生儿随机分为观察组(n=43)与对照组(n=43),对照组仅给予常规治疗,观察组在对照组基础上加用纳洛酮治疗,对比2组患儿神经功能及大脑血供变化。结果观察组有效率显著高于对照组(P<0.01);治疗后2组患儿NBNA评分均显著增高(P<0.01),且观察组治疗后>35分患儿显著多于对照组(P<0.01),2组患儿NSE及S100蛋白也较治疗前明显降低(P<0.01),且治疗后观察组显著低于对照组(P<0.01);2组患儿治疗后大脑中动脉 Vs、Vd均显著增加(P<0.01),且治疗后观察组显著高于对照组(P<0.01),2组血管阻力指数均显著降低(P<0.01),且治疗后观察组显著低于对照组(P<0.01)。结论纳洛酮用于新生儿重度窒息能够有效改善患儿神经功能状态及大脑血供,而脑血流灌注的增加将促进脑损伤及神经功能的恢复,疗效确切,值得推广。

Objective To investigate the influence of naloxone on the neurological function and cerebral blood supply of ne-onate with severe asphyxia. Methods Eighty-six neonate with severe asphyxia were randomly divided into the observation group (n=43 ,treated by conventional therapy combined with naloxone ) and control group (n=43 ,treated by conventional therapy). The neurological function and cerebral blood supply of two groups were compared.Results The effective rate of ob-servation group was significantly higher than that of control group (P<0.01);the cases of NBNA score>35 ,Vs and Vd of middle cerebral artery in the observation group were significantly increased and higher than these in the control group (P<0.01);the NSE and S100 protein and IR in the observation group were significantly decreased and lower than these in the con-trol group (P<0.01).Conclusion Naloxone can effectively improve the neurological function and cerebral blood supply in neo-nate with severe asphyxia ,which is worthy of applicaiton.
