目的:评价彩色多普勒超声(CDUS)与钼靶 X 线乳腺影像报告和数据系统(Breast Imaging Reporting and Data System,BI-RADS)对乳腺疾病临床诊断的应用价值。方法对111例经乳腺CDUS、钼靶X线检查患者资料进行回顾性分析,结合彩色多普勒超声(CDUS)与钼靶X线乳腺影像报告和数据系统(BI-RADS)分类,与手术或微创活检病理结果对照。结果在111例乳腺病变中,病理证实恶性65例(58.56%),乳腺CDUS分类诊断符合率83.08%(54/65),钼靶X线分类诊断符合率76.92%(50/65),而两者联合分类诊断符合率92.31%(60/65),后两者之间差异有统计学意义(P<0.05);良性46例(41.44%),乳腺CDUS分类诊断良性病变符合率80.43%(37/46),钼靶X线分类诊断符合率63.04%(29/46),而两者联合分类诊断符合率93.69%,后者比例明显高于前两者,差异有统计学意义(P<0.05)。乳腺CDUS分类诊断的准确性为81.98%(91/111),乳腺钼靶X 线分类诊断的准确性为71.17%(79/111),前者高于后者,但差异无统计学意义(P>0.05);CDUS+钼靶X线分类诊断联合诊断的准确性为93.69%(104/111),两者单独使用与CDUS+钼靶X线分类诊断联合比较,后者比例明显高于前两者,差异有统计学意义(P<0.05)。不同病理类型的乳腺癌在不同影像学诊断分类中的比例差异无统计学意义(P>0.05)。结论对于乳腺疾病的诊断,乳腺CDUS与钼靶X线检查分类诊断系统联合应用准确性更高,明显高于两者单独应用,联合使用可以尽量避免低估及漏诊。

Objective To evaluate the application value of Color Doppler Ultrasonography (CDUS), mammography and Breast Imaging Reporting and Data System(BI-RADS) classification for the diagnosis of breast disease. Methods The data of 111 patients examined by breast CDUS and mammography were retrospectively analyzed. The comparison and classification of the patients information were undertaken with CDUS, mammography and BI-RADS classification compared with the results of operation and biopsy pathology. Results Among 111 samples of breast lesions,65 samples were confirmed to be malignant by pathology (58.56%);diagnosis coincidence rate was 83.08%(54/65) by breast CDUS; diagnosis coincidence rate was 76.92%(50/65)by mammography. Diagnosis coincidence rate was 92.31%(60/65) by mammography in combination with breast ultra-sonography. There was significant difference between the latter two (P<0.05). 46 samples were benign. Classification diagnosis coincidence rate of benign breast lesions was 80.43%(37/46)by breast CDUS. Classification diagnosis coincidence rate was 71.17%(79/111) by mammography. Classification diagnosis coincidence rate was 93.69%by combination of both. The proportion of the latter one was obviously higher than the former two. There was significant difference between the latter two (P<0.05). The classification diagnosis accuracy rate was 81.98%(91/111)by breast CDUS. Compared every single with breast CDUS plus mam-mography classification diagnosis, the proportion of the latter one was obviously higher than the former two. There was signifi-cant difference (P<0.05). There was no significant difference among the proportions of the different pathological types of breast cancers (P>0.05). Conclusion For the diagnosis of breast disease, the accuracy rate is obviously higher by breast CDUS in combination with mammography classification diagnosis than either single. The combined use may as far as possible avoid the under-estimation and miss diagnosis.
