    【目的】探讨和分析对终末期肾病伴金刚烷胺药物性脑病患者应用血液透析滤过（HDF）治疗的临床效果及价值。【方法】选取2007年9月至2013年9月在本院接受透析治疗的终末期肾病伴金刚烷胺引起的药物性脑病患者51例为研究对象，随机分成研究组（26例）和对照组（25例），分别行 HDF和血液透析（HD ）治疗，用LC‐M S法测定血液透析前后患者的血浆金刚烷胺水平，并观察精神症状改善情况。【结果】两组患者透析前，血浆中金刚烷胺检测水平对比差异无显著性（ P＞0．05），行相应透析治疗后，研究组患者金刚烷胺水平较治疗前均明显下降，前后对比差异显著（ P ＜0．05），对照组治疗后血浆金刚烷胺水平有所下降，但下降不明显，前后对比无显著性差异（P ＞0．05）；治疗后研究组患者金刚烷胺水平较对照组明显下降，且差异有显著性（P ＜0．05）。两组患者治疗前，精神症状评分对比差异不明显（P ＞0．05），但透析治疗后，研究组精神症状评分较对照组明显下降，且差异有显著性（ P ＜0．05）。【结论】对终末期肾病合并金刚烷胺引发的药物性脑病患者行 HDF治疗，能明显降低血浆中金刚烷胺水平，有利于患者药物性脑病精神症状的改善及生活质量提升，值得在临床上推广。
    【목적】탐토화분석대종말기신병반금강완알약물성뇌병환자응용혈액투석려과（HDF）치료적림상효과급개치。【방법】선취2007년9월지2013년9월재본원접수투석치료적종말기신병반금강완알인기적약물성뇌병환자51례위연구대상，수궤분성연구조（26례）화대조조（25례），분별행 HDF화혈액투석（HD ）치료，용LC‐M S법측정혈액투석전후환자적혈장금강완알수평，병관찰정신증상개선정황。【결과】량조환자투석전，혈장중금강완알검측수평대비차이무현저성（ P＞0．05），행상응투석치료후，연구조환자금강완알수평교치료전균명현하강，전후대비차이현저（ P ＜0．05），대조조치료후혈장금강완알수평유소하강，단하강불명현，전후대비무현저성차이（P ＞0．05）；치료후연구조환자금강완알수평교대조조명현하강，차차이유현저성（P ＜0．05）。량조환자치료전，정신증상평분대비차이불명현（P ＞0．05），단투석치료후，연구조정신증상평분교대조조명현하강，차차이유현저성（ P ＜0．05）。【결론】대종말기신병합병금강완알인발적약물성뇌병환자행 HDF치료，능명현강저혈장중금강완알수평，유리우환자약물성뇌병정신증상적개선급생활질량제승，치득재림상상추엄。
    [Objective] To explore and analyze clinical efficacy and value of hemodiafiltration (HDF) for the treatment of patients with end‐stage renal disease accompanying by amantadine‐induced encephalopathy .[Meth‐ods]A total of 51 patient with end‐stage renal disease treated by dialysis in our hospital from Sept .2007 to Sept . 2013 were selected as the study subjects and randomly divided into the study group ( n=26) and control group( n=25) .Two group received HDF and dialysis ,respectively .LC‐MS was used to detect plasma amantadine in pa‐tients before and after hemodialysis .The improvement of mental symptoms was observed .[Results]There was no significant difference in plasma amantadine between two groups before dialysis ( P>0 .05) .Compared with before treatment ,plasma amantadine in the study group after the corresponding dialysis treatment was decreased obvious‐ly ,and there was significant different before and after treatment ( P <0 .05) .Before the treatment ,there was no obvious difference in mental symptom score between two groups .Compared with the control group ,the mental symptoms in the study group after dialysis were obviously decreased ,and there was significant difference ( P <0 .05) .[Conclusion] HDF for the treatment of patients with end‐stage renal disease accompanying by amantadine‐induced encephalopathy can obviously decrease plasma level of amantadine and is conducive to the improvement of mental symptoms and quality of life in patients with drug‐induced encephalopathy .Therefore ,it is worthy of clini‐cal promotion .
