【目的】采用Meta分析评价系统性红斑狼疮（SLE）患者中肿瘤的标准化发病率（SIR），评估SLE患者中总体肿瘤及某些实体肿瘤的风险性。【方法】检索 PubMed、中国学术期刊全文数据库（CNKI）获得已公开发表的国内外关于SLE患者与恶性肿瘤发病关系的文献，筛选出符合评价标准的文献，应用Stata11．0软件进行Meta分析，制作森林图并计算合并的标准化发病率及95％可信区间。【结果】SLE患者发生总体恶性肿瘤的风险较普通人群轻度升高（SIR＝1．28，95％ CI 1．00～1．62），发生血液系统恶性肿瘤（SIR＝2．93，95％ CI 1．83～4．68）、肝癌（SIR＝1．83，95％ CI 1．77～1．91）、肺癌（SIR＝1．55，95％ CI 1．40～1．73）、甲状腺癌（SIR＝2．22，95％ CI 2．11～2．34）的风险与普通人群比较显著增加，而发生宫颈癌（SIR ＝1．09，95％ CI 0．70～1．70）的风险较普通人群无明显增加。【结论】SLE患者中恶性肿瘤的发生率较普通人群有所增加，尤其是血液系统肿瘤、肝癌、肺癌、甲状腺癌的发病风险明显增加，因此在狼疮患者的随访过程中需警惕恶性肿瘤的发生。
【목적】채용Meta분석평개계통성홍반랑창（SLE）환자중종류적표준화발병솔（SIR），평고SLE환자중총체종류급모사실체종류적풍험성。【방법】검색 PubMed、중국학술기간전문수거고（CNKI）획득이공개발표적국내외관우SLE환자여악성종류발병관계적문헌，사선출부합평개표준적문헌，응용Stata11．0연건진행Meta분석，제작삼림도병계산합병적표준화발병솔급95％가신구간。【결과】SLE환자발생총체악성종류적풍험교보통인군경도승고（SIR＝1．28，95％ CI 1．00～1．62），발생혈액계통악성종류（SIR＝2．93，95％ CI 1．83～4．68）、간암（SIR＝1．83，95％ CI 1．77～1．91）、폐암（SIR＝1．55，95％ CI 1．40～1．73）、갑상선암（SIR＝2．22，95％ CI 2．11～2．34）적풍험여보통인군비교현저증가，이발생궁경암（SIR ＝1．09，95％ CI 0．70～1．70）적풍험교보통인군무명현증가。【결론】SLE환자중악성종류적발생솔교보통인군유소증가，우기시혈액계통종류、간암、폐암、갑상선암적발병풍험명현증가，인차재랑창환자적수방과정중수경척악성종류적발생。
[Objective] To evaluate the standardized incidence rate (SIRs) of malignant tumor in patients with systemic lupus erythematosus(SLE) by using Meta‐analysis ,and to assess the risk of overall malignancies and some solid malignancies in patients with SLE .[Methods]The literatures according with the evaluation cri‐terion were screened by searching the domestic and abroad published literatures about the relationship between SLE and the incidence of malignancies from PubMed and CNKI databases .Statl11 .0 software was used for Meta‐analysis .The forest map was made .The combined SIRs and 95% confidence interval(CI) were calculat‐ed .[Results]Compared with the general population ,the risk of the overall malignancies in SLE patients was slightly increased(SIR=1 .28 ,95% CI 1 .00~1 .62) ,and the risk of hematologic cancers (SIR=2 .93 ,95% CI 1 .83~4 .68) ,liver cancer(SIR=1 .83 ,95% CI 1 .77~1 .91) ,lung cancer(SIR=1 .55 ,95% CI 1 .40~1 .73) and thyroid cancer(SIR=2 .22 ,95% CI 2 .11~2 .34) were significantly increased ,but the risk of cervix cancer (SIR=1 .09 ,95% CI 0 .70~1 .70) was not obviously increased .[Conclusion]Compared with the general pop‐ulation ,the incidence of malignancies in patients with SLE is slightly increased ,especially the risk of hemato‐logic cancer ,liver cancer ,lung cancer and thyroid cancer are obviously increased .Therefore ,the incidence of malignancies should be cautioned during the follow up of patients with SLE .
