【目的】研究老年慢性阻塞性肺疾病(COPD)合并糖尿病(DM )的患者免疫功能变化及临床意义。【方法】65例年龄大于65岁的老年COPD稳定期患者分为COPD合并DM 组(35例)和COPD组(30例),同时选择20例健康老年人作为对照组,分别测定外周血细胞免疫和体液免疫指标。【结果】COPD+ DM 组 IgA、IgG、IgM、CD4+水平均低于COPD 组和对照组,CD8+水平高于COPD组和对照组,CD4+/CD8+倒置更为明显,差异均有统计学意义( P <0.05),而CD3+水平COPD组和COPD+DM组之间无显著性差异( P >0.05)。【结论】老年COPD合并DM的患者免疫功能低下,预防和控制DM对于提高COPD患者的免疫功能有重要意义。

【목적】연구노년만성조새성폐질병(COPD)합병당뇨병(DM )적환자면역공능변화급림상의의。【방법】65례년령대우65세적노년COPD은정기환자분위COPD합병DM 조(35례)화COPD조(30례),동시선택20례건강노년인작위대조조,분별측정외주혈세포면역화체액면역지표。【결과】COPD+ DM 조 IgA、IgG、IgM、CD4+수평균저우COPD 조화대조조,CD8+수평고우COPD조화대조조,CD4+/CD8+도치경위명현,차이균유통계학의의( P <0.05),이CD3+수평COPD조화COPD+DM조지간무현저성차이( P >0.05)。【결론】노년COPD합병DM적환자면역공능저하,예방화공제DM대우제고COPD환자적면역공능유중요의의。
[Objective] To explore the changes and significance of immune function in old patients with chronic ob‐structive pulmonary disease (COPD ) complicated by diabetes mellitus (DM ) .[Methods] A total of 65 old patients (aged more than 65 years old) with stable COPD were divided into COPD with DM group ( n=35) and COPD group( n=30) . Meanwhile ,20 healthy old people were chosen as the control group .Cellular immunity and humoral immunity indexes in peripheral blood were measured .[Results] IgA ,IgG ,IgM and CD4+ levels in COPD with DM group were lower than those in COPD group and control group ,while CD8+ level was higher than that in COPD group and control group ,and the inversion of CD4+ /CD8+ was more obvious ,and there were significant differences ( P<0 .05) .There was no signifi‐cant difference in CD3+ between COPD group and COPD with DM group ( P>0 .05) .[Conclusion]Elderly patients with COPD complicated by DM have immune dysfunction .The prevention and control of DM is important for improving the immune function in patients with COPD .
