目的：探讨华法林抗凝剂治疗非瓣膜性心房颤动(房颤)患者的影响因素。方法筛选我院2011年3月至2013年3月接诊的58例华法林抗凝治疗非瓣膜病心房颤动患者的临床资料。对华法林抗凝治疗的影响因素采取非条件多因素Logistic回归分析。结果患者的年龄(>75岁)、疾病类型(如高血压、冠心病、慢性肺疾病、心肌病等)、临床医师的治疗决策(对华法林的认识全面，不推荐华法林的治疗)以及患者因素(拒绝华法林的治疗，或是INR检测不方便)等都可以影响华法林抗凝治疗非瓣膜病心房颤动的疗效，但是只有患者的年龄和冠心病(OR=13.38)对华法林治疗的影响差异有统计学意义(P<0.05)。结论影响华法林抗凝剂治疗非瓣膜性心房颤动(房颤)的因素很多，其中患者的年龄、疾病类型可显著影响华法林抗凝治疗非瓣膜病心房颤动的疗效。

Objective To explore the influence factors of warfarin anticoagulant therapy for treating nonvalvu-lar atrial fibrillation (AF). Methods Fifty-eight patients with nonvalvular atrial fibrillation underwent warfarin anti-coagulant therapy in our hospital from March 2011 to March 2013 were enrolled in this study. Influence factors of war-farin anticoagulant therapy were analyzed by unconditional multi-factor logistic regression. Results The age of patients (over 75 years), the types of diseases (such as hypertension, coronary heart disease, chronic lung disease and cardiomyopathy), clinicians' treatment decisions (fully understand warfarin and do not recommend warfarin anticoagulant therapy) and patients' factors (reject warfarin anticoagulant therapy or inconvenient with INR detec-tion) might be the influence factors of warfarin anticoagulant therapy for treating nonvalvular atrial fibrillation, but, only the age of patients and coronary heart disease (OR=13.38) had significant effects on the warfarin antico-agulant therapy (P<0.05). Conclusion There are many influence factors of warfarin anticoagulant therapy in the treatment of nonvalvular atrial fibrillation, among which the age of patients and types of diseases can significantly af-fect warfarin anticoagulant therapy of nonvalvular atrial fibrillation.
