目的:了解肇庆地区健康体检人群EB病毒的感染状况,为临床鼻咽癌的早期诊断提供依据。方法选取2011-2012年在本院进行健康体检的8550例健康体检者作为观察组,以同期入院治疗的300例鼻咽癌患者作为对照组,采用酶联免疫吸附试验(ELISA)检测并比较两组患者EB病毒抗体的阳性情况。结果(1)观察组患者共检查出EB-VCA-IgA阳性324例,阳性率为3.79%,EB-NA1-IgA阳性140例,阳性率为1.64%,EB-EA-IgG阳性150例,阳性率为1.75%;上述三项指标均明显低于对照组,差异有统计学意义(P<0.05);(2) EB-VCA-IgA、EB-NA1-IgA和EB-EA-IgG的阳性情况均与鼻咽癌的发生呈正相关关系,且优势比OR值分别为7.875、4.998、4.823。结论本地区健康体检人群EB-VCA-IgA的阳性率最高,EB-EA-IgG阳性率次之,EB-NA1-IgA阳性率最低,且EB-VCA-IgA阳性患者发生鼻咽癌的风险更高,对EB病毒抗体进行检测具有重要的临床意义。

Objective To investigate the prevalence of EB virus infection in healthy people in Zhaoqing city and provide evidence for early diagnosis of nasopharyngeal carcinoma. Methods A total of 8 550 people underwent health examination were selected as observation group. And 300 nasopharyngeal carcinoma patients were enrolled as control group. EB antibodies in the two groups were detected by ELISA and then compared. Results (1) In the obser-vation group, the positive rates of EB-VCA-IgA, EB-NA1-IgA and EB-EA-IgG were 3.79%(324 cases), 1.64%(140 cases) and 1.75%(150 cases) respectively, which were all significantly lower than those in the control group (P<0.05);(2) EB-VCA-IgA, EB-NA1-IgA and EB-EA-IgG were positively correlated with nasopharyngeal carcinoma, where OR values were 7.875, 4.998, and 4.823 respectively. Conclusion The positive rate of EB-VCA-IgA in healthy peo-ple in this area is the highest, followed by the positive rate of EB-EA-IgG, and the positive rate of EB-NA1-IgA is the lowest. Patients with positive EB-VCA-IgA have a higher risk of nasopharyngeal carcinoma, therefore it's important for detecting EB antibody.
