目的:探讨Fascin在大肠癌组织中的表达及临床意义。方法采用免疫组化Elivision法检测Fascin蛋白在130例大肠癌组织和30例大肠正常组织中的表达情况,分析其与临床病理特征的关系及对患者预后的影响。结果 Fascin蛋白在大肠癌组织中的阳性率为54.6%,明显高于正常大肠组织中的阳性率(16.7%)。在大肠癌中,Fascin蛋白的表达与组织学分级、浆膜侵犯、淋巴结转移及临床分期有关(P<0.05),与患者的年龄、性别及肿瘤大小无关(P>0.05)。多因素Cox回归分析显示:淋巴结转移、临床分期及Fascin蛋白表达为影响患者预后的独立危险因素(P<0.05)。Kaplan-Meier生存曲线显示,Fascin蛋白的阳性表达组的生存时间低于阴性组(P<0.05)。结论 Fascin蛋白的表达上调可能促进大肠癌的侵袭转移,对评价大肠癌的生物学行为和预后具有一定的参考价值。

Objective To investigate the expression and clinical significance of Fascin in colorectal carcino-ma. Methods With Elivision immunohistochemistry, the expression of Fascin was detected in 130 patients of colorectal carcinoma and 30 patients of normal tissue. Its clinicopathological characteristics and prognostic signifi-cance was analyzed. Results The positive expression rate of Fascin was 54.6%in colorectal carcinoma, which was significantly higher than that 16.7%in normal tissus. In colorectal carcinoma, the expression of Fascin was correlated with degree of differentiation, serosal invasion, lymph node metastasis and dukes stage, and irrelevant with age, gen-der, size of tumor. Multivariate Cox regression analysis showed that lymph node metastasis, dukes stage and Fascin over expression were independent risk factors affecting the prognosis of patients. The plotted survival curves by the Kaplan-Meier method demonstrated that overall survival was significantly shorter in patients with high expression than in patients with low expression of Fascin protein. Conclusion The expression of Fascin may ultimately promote tumor invasion and metastasis of colorectal carcinoma. It can be used as important parameters for judgement of biolog-ical behavior and prognosis.
