目的：探讨肱骨近端交锁髓内钉治疗肱骨外科颈骨折临床效果。方法应用肱骨近端交锁髓内钉治疗14例肱骨近端二部分外科颈骨折和9例三部分大结节骨折患者。结果患者均获随访，时间12~28个月，骨折均骨性愈合。按照Constant-Murley肩关节评分标准评价为65~95分，其中优12例，良7例，一般4例。结论应用肱骨近端交锁髓内钉治疗肱骨近端二部分外科颈骨折和三部分大结节骨折，减少了软组织损伤，可早期行关节功能锻炼，是一种安全有效治疗方法。
목적：탐토굉골근단교쇄수내정치료굉골외과경골절림상효과。방법응용굉골근단교쇄수내정치료14례굉골근단이부분외과경골절화9례삼부분대결절골절환자。결과환자균획수방，시간12~28개월，골절균골성유합。안조Constant-Murley견관절평분표준평개위65~95분，기중우12례，량7례，일반4례。결론응용굉골근단교쇄수내정치료굉골근단이부분외과경골절화삼부분대결절골절，감소료연조직손상，가조기행관절공능단련，시일충안전유효치료방법。
Objective To investigate the clinical effect of intramedullary interlocking nail in treatment of proximal humeral neck fracture. Methods Fourteen cases of proximal humeral neck two-part fracture and nine cases of three-part fracture with humeral great tubercle fractures were treated by intramedullary interlocking nail fixation. Results All cases were followed up for 12~28 months. All fractures were bony healing. According to Constant-Murley shoul-der system, the scores were 65~95. 12 cases were excellent, 7 good and 4 fair. Conclusions Intramedullary inter-locking nail internal fixation in treating proximal humeral fractures have advantages of simple operation, and strong fixation, which reduces the blood of bone damage and peri-operative bleeding, and also can let shoulder function ex-ercise early. It is good for fracture healing.
