目的:观察保留髓内钉、附加锁定钢板微创治疗股骨骨折髓内钉固定术后骨不连的效果。方法对17例股骨骨折髓内钉固定术后骨不连患者采用保留髓内钉、附加锁定钢板微创治疗。术后定期随访,观察患肢活动、负重及骨痂生长情况。结果17例均获随访,时间6~18(9±2)个月,患者均获骨性愈合。无感染、钢板螺钉松动、断裂等现象等。结论保留髓内钉、附加锁定钢板微创治疗股骨骨折髓内钉固定术后骨不连,手术创伤小,骨膜可不用剥离,血运破坏小,固定牢靠。结合早期功能锻练,可促进骨折愈合,是一种简单有效的治疗方法。

목적:관찰보류수내정、부가쇄정강판미창치료고골골절수내정고정술후골불련적효과。방법대17례고골골절수내정고정술후골불련환자채용보류수내정、부가쇄정강판미창치료。술후정기수방,관찰환지활동、부중급골가생장정황。결과17례균획수방,시간6~18(9±2)개월,환자균획골성유합。무감염、강판라정송동、단렬등현상등。결론보류수내정、부가쇄정강판미창치료고골골절수내정고정술후골불련,수술창상소,골막가불용박리,혈운파배소,고정뢰고。결합조기공능단련,가촉진골절유합,시일충간단유효적치료방법。
Objective To analyze the effect of using minimally invasive augmentation plating for femoral shaft nonun-ions after intramedullary nail fixation. Methods 17 patients with femoral shaft nonunions after intramedullary nail fixation were selected. Minimally invasive augmentation plating was used to treat these patients. All the patients were followed regularly, the limb activity, weight bearing capability and callus growth were observed. Results 17 patients were followed up for 6~18(9 ± 2) months,osseous healing was achieved in all patients,without the phenomenon such as infection, breakage of steel screw or loosening, etc. Conclusions It is a good method to use minimally invasive augmentation plating to treat femoral shaft nonunions after intramedullary nail fixation. The operative trauma and blood supply damage are rather small, the periosteum is well preserved, and the fracture is healed. Fixation stability allows the patients with early functional exercise, promote healing of fracture. It is a simple and effective treatment method.
