目的：探讨腰椎终板Modic I型改变对伴下腰痛的腰椎间盘突出症患者手术疗效的影响。方法回顾性分析2008年3月至2012年l1月收治的伴下腰痛的腰椎间盘突出症患者63例,分为两组：A组伴终板Modic I型改变,B组无终板Modic改变,均行单节段开窗减压髓核摘除术。采用疼痛视觉模拟评分( VAS)、Oswestry功能障碍指数( ODI)评估两组患者手术后的临床效果,评价终板 Modic I型改变对患者术后下腰痛改善的影响。结果术前 A、B两组的VAS、ODI评分差异均无统计学意义(P>0．05)。术后两组患者坐骨神经痛症状均明显缓解或完全消除。术后随访VAS及ODI评分两组差异均有统计学意义(P<0．05)。结论腰椎间盘突出症术后下腰痛的改善与终板Modic I型改变存在相关性。

Objective To evaluate the outcomes of lumbar endplate Modic degeneration for the treatment of lumbar disc herniation and low back pain. Methods From March 2008 to November 2012,63 patients of L4/5 disc herniation with low back pain were analyzed retrospectively. All patients had unilateral fenestration and discectomy. The clinical effects were evaluated by the scores of visual analog scale ( VAS ) and Oswestry disability index ( ODI ) . Results There was no difference between two groups before operation with respect to VAS and ODI. The symptoms of all the patients were relieved postoperatively. Postoperative follow-up of VAS and ODI scores between the two groups was of statistical significance. Conclusion There is a correlation between the recovery of low back pain and Modic I changes.
