Complex upper urinary tract calculi%Holmium laser lithotripsy%Ureteroscopy
目的：探讨经皮肾输尿管镜钬激光碎石治疗复杂性上尿路结石的临床疗效。方法：选取笔者所在医院2009年1月-2013年12月收治的201例219侧复杂性肾、输尿管上段结石患者，其中包括肾盂肾盏多发结石120侧，鹿角状结石63侧，输尿管上段大结石36例，以上患者均采用钬激光碎石术进行治疗，对患者的手术时间、结石排净率以及并发症情况进行观察分析。结果：本组201例219侧复杂性上尿路结石患者的平均手术时间为(69.1±12.7)min，一次性结石排净率为59.0%(129/219)，总结石排净率为88.0%(192/219)，术中出血量为(85.4±9.6)ml，术后9例患者出现一过性高热，无一例严重并发症出现。结论：采用经皮肾输尿管镜钬激光碎石治疗复杂性上尿路结石的疗效确切，手术时间短，出血量少，结石排净率高，值得在临床上推广应用。
目的：探討經皮腎輸尿管鏡鈥激光碎石治療複雜性上尿路結石的臨床療效。方法：選取筆者所在醫院2009年1月-2013年12月收治的201例219側複雜性腎、輸尿管上段結石患者，其中包括腎盂腎盞多髮結石120側，鹿角狀結石63側，輸尿管上段大結石36例，以上患者均採用鈥激光碎石術進行治療，對患者的手術時間、結石排淨率以及併髮癥情況進行觀察分析。結果：本組201例219側複雜性上尿路結石患者的平均手術時間為(69.1±12.7)min，一次性結石排淨率為59.0%(129/219)，總結石排淨率為88.0%(192/219)，術中齣血量為(85.4±9.6)ml，術後9例患者齣現一過性高熱，無一例嚴重併髮癥齣現。結論：採用經皮腎輸尿管鏡鈥激光碎石治療複雜性上尿路結石的療效確切，手術時間短，齣血量少，結石排淨率高，值得在臨床上推廣應用。
목적：탐토경피신수뇨관경화격광쇄석치료복잡성상뇨로결석적림상료효。방법：선취필자소재의원2009년1월-2013년12월수치적201례219측복잡성신、수뇨관상단결석환자，기중포괄신우신잔다발결석120측，록각상결석63측，수뇨관상단대결석36례，이상환자균채용화격광쇄석술진행치료，대환자적수술시간、결석배정솔이급병발증정황진행관찰분석。결과：본조201례219측복잡성상뇨로결석환자적평균수술시간위(69.1±12.7)min，일차성결석배정솔위59.0%(129/219)，총결석배정솔위88.0%(192/219)，술중출혈량위(85.4±9.6)ml，술후9례환자출현일과성고열，무일례엄중병발증출현。결론：채용경피신수뇨관경화격광쇄석치료복잡성상뇨로결석적료효학절，수술시간단，출혈량소，결석배정솔고，치득재림상상추엄응용。
Objective:To investigate the clinical effect of percutaneous renal ureteroscope holmium laser lithotripsy in the treatment of complex upper urinary tract calculi.Method:201 patients (219 sides) with complex renal and ureteral stones admitted to our hospital from January 2009 to December 2013 were selected,including the pelvis and multiple renal calculi in 120 sides,63 sides were staghorn calculus,36 cases were upper ureteral stones,the patients were treated by holmium laser lithotripsy for treatment,the operation time,the lithagogue rate and complications of all patients were observed and analyzed.Result:In 201 patients(219 sides) with complex upper urinary stone,the average operation time was (69.1±12.7)min,the disposable lithagogue rate was 59.0%(129/219), the total lithagogue rate was 88.0%(192/219),the amount of bleeding was (85.4±9.6)ml,9 patients experienced high heat postoperative,with no serious complications.Conclusion:The curative effect of percutaneous renal ureteroscope holmium laser lithotripsy in treatment of complex upper urinary tract calculi is exact,shorter operation time,less bleeding,the lithagogue rate is high,is worthy of clinical popularization and application.
