目的:观察肥胖对P波最大时限(Pmax)及其离散度(Pd)的影响,探讨肥胖导致房颤的可能机制。方法选取肥胖患者40例,按照年龄、性别匹配的原则选取20例作为对照组;描记12导联心电图,测量P波最大时限(Pmax)、P波最小时限(Pmin)及P波离散度(Pd)。结果两组间年龄、性别、高血压、糖尿病比例及血脂水平无显著性差异;肥胖组体重指数及左房直径较对照组明显增加(P<0.01)、P波最大时限和P波离散度较对照组明显延长(P<0.01)。结论肥胖可导致P波最大时限和P波离散度显著延长,并可能参与房颤的发生。

목적:관찰비반대P파최대시한(Pmax)급기리산도(Pd)적영향,탐토비반도치방전적가능궤제。방법선취비반환자40례,안조년령、성별필배적원칙선취20례작위대조조;묘기12도련심전도,측량P파최대시한(Pmax)、P파최소시한(Pmin)급P파리산도(Pd)。결과량조간년령、성별、고혈압、당뇨병비례급혈지수평무현저성차이;비반조체중지수급좌방직경교대조조명현증가(P<0.01)、P파최대시한화P파리산도교대조조명현연장(P<0.01)。결론비반가도치P파최대시한화P파리산도현저연장,병가능삼여방전적발생。
Objective To study the effects of obesity on Maximum P-wave duration (Pmax) and P wave dispersion (Pd) in order to evaluate the potential mechanisms for atrial fibrillation (AF) development in Chinese obesity subjects Methods The study population consisted of 40 obese subjects and 20 age-and sex-matched normal weight controls. Pmax, minimum Pwave duration (Pmin), and Pd were carefully measured using a 12-lead electrocardiogram. Results There were no significant differences between the two groups regarding age, sex, history of hypertension or diabetes, and hyperlipidemia. Compared to controls, BMI, left atrial diameter (LAD) and interventricular septal thickness (IVS) were increased (P<0.05), while Pmax and Pd (P<0.01) were significantly prolonged in the obese group. Conclusion Our data suggest that obesity is associated with increased Pmax and Pd, may be associated with the development of AF.
