目的：评价经椎弓根椎体内楔形闭合截骨术治疗胸腰椎后凸畸形的临床疗效。方法23例胸腰椎后凸畸形患者，行椎弓根楔形闭合截骨矫正术，其中陈旧性胸腰椎骨折14例；先天性半椎体畸形5例；强直性脊柱炎4例。观察术前、术后6个月及末次随访矢状面Cobb角、视觉模拟量表（ visual analog scale ， VAS）评分和Oswestry功能障碍指数（ Oswestry disability index ， ODI）的变化。结果平均随访34个月，术后后凸Cobb角与术前比较差异有统计学意义（P＜0．05），末次随访时后凸Cobb角与术后比较无明显丢失（P＞0．05）；术后腰痛及末次随访VAS评分与术前比较差异有统计学意义（P＜0．05），术后6个月随访ODI与术前比较差异有统计学意义（P＜0．05）。结论经椎弓根椎体内楔形闭合截骨术治疗胸腰椎后凸畸形能获得满意的临床疗效。
목적：평개경추궁근추체내설형폐합절골술치료흉요추후철기형적림상료효。방법23례흉요추후철기형환자，행추궁근설형폐합절골교정술，기중진구성흉요추골절14례；선천성반추체기형5례；강직성척주염4례。관찰술전、술후6개월급말차수방시상면Cobb각、시각모의량표（ visual analog scale ， VAS）평분화Oswestry공능장애지수（ Oswestry disability index ， ODI）적변화。결과평균수방34개월，술후후철Cobb각여술전비교차이유통계학의의（P＜0．05），말차수방시후철Cobb각여술후비교무명현주실（P＞0．05）；술후요통급말차수방VAS평분여술전비교차이유통계학의의（P＜0．05），술후6개월수방ODI여술전비교차이유통계학의의（P＜0．05）。결론경추궁근추체내설형폐합절골술치료흉요추후철기형능획득만의적림상료효。
Objective To evaluate the safety and efficacy of the pedicle subtraction osteotomy ( PSO) in the treatment of thoracolumbar kyphosis deformity .Methods A total of 23 patients of thoracolumbar kyphosis underwent PSO , among whom 14 cases were post-traumatic kyphosis , 5 cases were congenital hemivertebrae deformity and 4 cases were ankylosing spondyli-tis.The kyphotic Cobb’s angle, visual analog scale (VAS) scores of back pain and Oswestry disability index (ODI) preop-eraitve, postoperative 6 months and at the final follow-up were accessed.Results Mean follow-up time was 34 months.There was significantly difference between pre-and postoperative Cobb’s angels(P <0.05).The difference of Cobb’s angle between the final follow-up and post-operation wasn’t statistically significant(P>0.05).The VAS scores of back pain post-operative and at the final follow-up were significantly difference from that of pre-operation(P<0.05).The postoperative 6 months ODI was significantly different from that of pre-operation(P<0.05).Conclusion PSO is an effective and safe proce-dure to correct kyphosis, which has obtained satisfactory clinical outcome .
