目的:观察钢丝球囊锚定技术在严重弯曲伴钙化的冠状动脉病变介入治疗中的应用效果。方法选择56例经冠脉造影明确为严重弯曲伴钙化病变,在强支撑指引导管、病变部位充分球囊预扩张和双钢丝等技术基础上运用分支锚定技术行支架植入失败的患者,采用钢丝球囊锚定技术,评价其临床效果并观察并发症发生情况。结果47例采用钢丝球囊锚定技术成功植入支架,手术成功率达83.93%,其中2例术中发生慢血流现象,予以冠脉内注射盐酸替罗非班和地尔硫后恢复血流;另外9例因钙化严重,经血管内超声确认系360度内膜下严重钙化,改行冠脉内旋磨术后成功植入支架。结论钢丝球囊锚定技术处理严重弯曲钙化冠状动脉病变安全、有效。
目的:觀察鋼絲毬囊錨定技術在嚴重彎麯伴鈣化的冠狀動脈病變介入治療中的應用效果。方法選擇56例經冠脈造影明確為嚴重彎麯伴鈣化病變,在彊支撐指引導管、病變部位充分毬囊預擴張和雙鋼絲等技術基礎上運用分支錨定技術行支架植入失敗的患者,採用鋼絲毬囊錨定技術,評價其臨床效果併觀察併髮癥髮生情況。結果47例採用鋼絲毬囊錨定技術成功植入支架,手術成功率達83.93%,其中2例術中髮生慢血流現象,予以冠脈內註射鹽痠替囉非班和地爾硫後恢複血流;另外9例因鈣化嚴重,經血管內超聲確認繫360度內膜下嚴重鈣化,改行冠脈內鏇磨術後成功植入支架。結論鋼絲毬囊錨定技術處理嚴重彎麯鈣化冠狀動脈病變安全、有效。
목적:관찰강사구낭묘정기술재엄중만곡반개화적관상동맥병변개입치료중적응용효과。방법선택56례경관맥조영명학위엄중만곡반개화병변,재강지탱지인도관、병변부위충분구낭예확장화쌍강사등기술기출상운용분지묘정기술행지가식입실패적환자,채용강사구낭묘정기술,평개기림상효과병관찰병발증발생정황。결과47례채용강사구낭묘정기술성공식입지가,수술성공솔체83.93%,기중2례술중발생만혈류현상,여이관맥내주사염산체라비반화지이류후회복혈류;령외9례인개화엄중,경혈관내초성학인계360도내막하엄중개화,개행관맥내선마술후성공식입지가。결론강사구낭묘정기술처리엄중만곡개화관상동맥병변안전、유효。
Objective To observe the effect of application wire-balloon-anchoring technique in severe bending with calci-fied coronary artery disease.Methods To observe the complications and evaluate the clinical effects of wire-balloon-anchoring technique,5 6 patients with severe bending with calcified lesions diagnosed by coronary angiography were adopted the technique after failing to application of strong support of guiding catheter,full balloon pre-expansion and double-wire-branch-anchoring technique.Results Forty-seven patients with wire-balloon-anchoring technique successful stenting,success rate was 83.93%. Two patients with slow blood flow phenomenon restored blood flow after intravascular inj ection with tirofiban and diltiazem. Another nine patients with extreme calcification of 360 degree of intima diagnosed by intravascular Ultrasound (IVUS)were successfully stented after switching to apply rotational atherectomy.Conclusion Wire-balloon-anchoring technique is a safe and effective means of dealing with severe bending and calcified coronary artery lesions.
