目的：评价手术坚强内固定在儿童下颌骨骨折中的临床效果。方法对28例下颌骨骨折患儿应用紧贴下颌下缘上方的小型钛板双皮质固定+单颌牙弓夹板固定手术切开复位内固定术。结果28例患儿下颌骨骨折均获得良好的解剖复位，骨折Ⅰ期愈合，面形、开口型均正常，张闭口活动正常，口内咬牙合关系良好，未见骨折部位恒牙萌出异常，患儿无不良反应。结论儿童下颌骨骨折采用紧贴下颌下缘上方的小型钛板双皮质坚固内固定联合牙弓夹板固定术，避免了下颌骨张力带钛板固定对恒牙胚的损伤和下颌骨应力带的扭转，不影响颏顶弹性帽固定对下颌的制动。患儿早期即可进食和进行功能锻炼，减少并发症的发生，对下颌骨的整复固定和功能恢复效果良好。
목적：평개수술견강내고정재인동하합골골절중적림상효과。방법대28례하합골골절환인응용긴첩하합하연상방적소형태판쌍피질고정+단합아궁협판고정수술절개복위내고정술。결과28례환인하합골골절균획득량호적해부복위，골절Ⅰ기유합，면형、개구형균정상，장폐구활동정상，구내교아합관계량호，미견골절부위항아맹출이상，환인무불량반응。결론인동하합골골절채용긴첩하합하연상방적소형태판쌍피질견고내고정연합아궁협판고정술，피면료하합골장력대태판고정대항아배적손상화하합골응력대적뉴전，불영향해정탄성모고정대하합적제동。환인조기즉가진식화진행공능단련，감소병발증적발생，대하합골적정복고정화공능회복효과량호。
Objective To assess the clinical effect of surgical reposition and rigid internal fixation for the treatment of children mandibular fracture.Methods Twenty-eight cases of children mandibular fracture were repaired by open reduction and internal fixation of application of the small cortical titanium plate fixed double cortex of close to the top of the lower edge of the mandible+ single jaw arch splint fixation.Results Twenty-eight cases of children with mandibular fractures had received good anatomic reduction and primary healing of fracture with normal face,open type,open-closed activities and good intraoral occlusal relationship. There were no abnormal eruption of permanent teeth fracture site and discomfort.Conclusion Children man-dibular fractures were corrected by a small titanium plate internal fixation combined with dual cortex sturdy arch splint fixation close to the lower edge of the upper jaw which avoid tension with titanium plate fixation of mandibular permanent tooth damage,stress torsional of mandibular and on the chin on the top elastic cap fixed jaw braking,the children can eat and early functional exercises to reduce the inci-dence of complications of mandibular fixed restoration and functional recovery to good effect.
