目的:探讨磁共振动态增强扫描定量参数在胰腺癌中的应用价值。方法用3.0T 磁共振对病理证实的27例胰腺癌患者进行动态增强扫描,图像通过 Jims 软件的 Toft with Vp 模型分别计算病灶和正常胰腺组织的定量参数:K trans ,k ep ,Ve ,Vp ,并应用 SPSS17.0软件进行单向方差分析。结果胰腺癌的 K trans 值、k ep 值、Ve 值、Vp 值分别为:(0.303±0.321)min,(1.387±1.486)min,(25.07±10.98)%和(3.420±4.692)%;而正常胰腺组织的 K trans 值、k ep 值、Ve 值、Vp 值分别为:(1.235±0.777)min,(9.277±7.996)min,(17.89±8.882)%,(7.196±6.704)%,胰腺癌及正常胰腺组织的各参数间均存在显著的统计学差异(F 值分别为33.188,25.414,6.984,5.78,P 值均<0.05)。结论胰腺癌磁共振动态增强扫描定量参数能够准确反映病灶血流灌注及微循环变化,有助于不典型病变的鉴别诊断。

목적:탐토자공진동태증강소묘정량삼수재이선암중적응용개치。방법용3.0T 자공진대병리증실적27례이선암환자진행동태증강소묘,도상통과 Jims 연건적 Toft with Vp 모형분별계산병조화정상이선조직적정량삼수:K trans ,k ep ,Ve ,Vp ,병응용 SPSS17.0연건진행단향방차분석。결과이선암적 K trans 치、k ep 치、Ve 치、Vp 치분별위:(0.303±0.321)min,(1.387±1.486)min,(25.07±10.98)%화(3.420±4.692)%;이정상이선조직적 K trans 치、k ep 치、Ve 치、Vp 치분별위:(1.235±0.777)min,(9.277±7.996)min,(17.89±8.882)%,(7.196±6.704)%,이선암급정상이선조직적각삼수간균존재현저적통계학차이(F 치분별위33.188,25.414,6.984,5.78,P 치균<0.05)。결론이선암자공진동태증강소묘정량삼수능구준학반영병조혈류관주급미순배변화,유조우불전형병변적감별진단。
Objective To investigate the value of dynamic contrast-enhanced quantitative parameters of pancreatic cancer at 3.0T MR.Methods 27 patients with pathologically proved pancreatic cancers were underwent DCE-MR at a 3.0 T scanner.A"Toft with Vp" model was used to quantify K trans ,k ep ,Ve and Vp in the pancreatic cancer and normal pancreatic tissues.All parameters among different tissues were analyzed and compared by SPSS1 7.0.Results The K trans 、k ep 、Ve 、Vp values of pancreatic cancer were(0.303± 0.321)min,(1.387±1.486)min,(25.07±10.98)%and(3.420±4.692)% respectively ,while those values of normal pancreatic tis-sue were (1.235±0.777)min,(9.277 ± 7.996 )min,(1 7.89 ± 8.882 )%,and(7.1 96 ± 6.704)%,respectively.The differences be-tween the four parameters of pancreatic cancer and normal pancreatic tissue were statistically significant(F =33.188,25.414,6.984, 5.78,P <0.05).Conclusion Quantitative parameters of DCE-MRI accurately reflect changes in tumor blood perfusion and microcir-culation,they may be helpful to differentiate the atypical lesion.
