目的：探讨经会阴饱和穿刺对前列腺癌的诊断价值。方法184例符合入选标准的患者随机分为观察组和对照组。观察组96例采用经会阴饱和穿刺活检，对照组88例采用经直肠穿刺活检，对比分析2组患者的穿刺活检阳性率和安全性。结果观察组的总阳性率比对照组高，差异有统计学意义（P＜0．05）。当PSA＜10．0μg/L和≥10．0～20．0μg/L时，观察组的阳性率比对照组高，差异有统计学意义（P＜0．05）；当PSA＞20．0～50．0μg/L和＞50．0μg/L时，观察组的阳性率与对照组相比无显著差异（P＞0．05）。观察组的发热、尿路感染、肉眼血尿、血便、直肠出血这些并发症的发生率均比对照组低，差异有统计学意义（ P＜0．05）。而观察组的尿潴留的发生率和对照组比较，差异无统计学意义（P＞0．05）。结论经会阴饱和穿刺活检能提高前列腺癌的检出率，并发症发生率低，值得临床推广。
목적：탐토경회음포화천자대전렬선암적진단개치。방법184례부합입선표준적환자수궤분위관찰조화대조조。관찰조96례채용경회음포화천자활검，대조조88례채용경직장천자활검，대비분석2조환자적천자활검양성솔화안전성。결과관찰조적총양성솔비대조조고，차이유통계학의의（P＜0．05）。당PSA＜10．0μg/L화≥10．0～20．0μg/L시，관찰조적양성솔비대조조고，차이유통계학의의（P＜0．05）；당PSA＞20．0～50．0μg/L화＞50．0μg/L시，관찰조적양성솔여대조조상비무현저차이（P＞0．05）。관찰조적발열、뇨로감염、육안혈뇨、혈편、직장출혈저사병발증적발생솔균비대조조저，차이유통계학의의（ P＜0．05）。이관찰조적뇨저류적발생솔화대조조비교，차이무통계학의의（P＞0．05）。결론경회음포화천자활검능제고전렬선암적검출솔，병발증발생솔저，치득림상추엄。
Objective To investigate the value of transperineal saturation prostate biopsy in the diagnosis of prostate cancer.Methods 184 eligible patients were randomly divided into the observation group and the control group .96 cases in the observation group received transperineal saturation prostate biopsy ,while 88 cases in the control group were treated by transrectal biopsy.The positive rate and safety of biopsy were compared and analyzed between the 2 groups.Results The positive rate in the observation group was higher than that of the control group ,the differences between the 2 groups had statistical significance .( P<0.05).In cases with PSA<10.0μg/L and≥10.0~20.0μg/L,the observation group had a higher positive rate than that of the control group,the difference had statistical significance (P<0.05).In cases with PSA >20.0~50.0 μg/L and >50.0 μg/L, the differences between the 2 groups were not statistically significant(P>0.05).The incidence of complications(including fever, urinary tract infection ,hematuria ,blood in stool and rectal bleeding ) in the observation group was significantly higher than those of the control group(P<0.05).As for retention of urine,the comparasion of incidence had no statistical significance (P>0.05). Conclusion Transperineal saturation prostate biopsy can improve the detection rate of prostate cancer ,the incidence of complica-tions is comparatively low ,and it is worthy of clinical promotion .
