目的:了解两种不同他汀类药物对不稳定型心绞痛患者的C-反应蛋白( hs-CRP),白介素-18( IL-18),基质蛋白酶-9(MMP-9)和血脂浓度的影响。方法选择2010年6月至2013年6月收治的60例不稳定型心绞痛患者作为研究对象。随机数字法分为两组,A组,30例,在常规治疗的基础上加用阿托伐他汀进行治疗;B组,30例,在常规治疗的基础上加用氟伐他汀进行治疗。测定并记录两组患者hs-CRP,IL-18,MMP-9,甘油三酯( TG),总胆固醇( TC),低密度脂蛋白-胆固醇( LDL-C),高密度脂蛋白-胆固醇( HDL-C),并进行对比分析。结果治疗后两组患者hs-CRP,IL-18,MMP-9均较治疗前显著降低,具有显著统计差异( P <0.05),但组间治疗后水平无显著统计差异( P >0.05);治疗后两组患者的TG,TC,LDL-C,HDL-C均较治疗前明显改善,具有显著统计差异( P <0.05),组间治疗后比较,A组TC水平较B组降低更明显,其余指标TG、LDL-C和HDL-C下降程度相比无显著统计差异( P <0.05);从不良反应发生率相比,两组在胃肠道症状和肝功能异常上相比具有显著统计差异( P <0.05),而在急性冠状动脉病变上无显著统计差异( P >0.05)。结论阿托伐他汀和氟伐他汀对降低不稳定型心绞痛患者的炎症反应和血脂均有一定疗效,虽然有不同程度的不良反应发生,但发生率都较低,在可控范围之内,因此两种他汀类药物是安全的,可靠的。

Objective To understand the influence of the C- reaction protein in patients with unstable angina pectoris of operation( hs-CRP),interleukin-18(IL-18),matrix metalloproteinase-9(MMP-9)and blood lipid concentration of two kinds of statins postoperative inflammatory factors. Methods 60 cases of unstable angina pectoris patients were treated in our hospital from 2010 June to 2013 June. All the 60 cases were included as the research object. All the cases were randomly divided into two groups,group A( including 30 cases)on the basis of con-ventional therapy plus treatment with atorvastatin,B group( including 30 cases)on the basis of conventional therapy plus treatment with fluvasta-tin. The hs-CRP,IL-18,MMP-9,TG,TC,LDL-C,HDL-C were measured,recorded,compared and analyzed. Results After treatment, hs-CRP,IL-18,and MMP-9 were therapy significantly less than before treatment,with significant statistical difference( P <0. 05). but there was no significant statistical difference between the two groups after the treatment( P >0. 05). The TG,TC,LDL-C,HDL-C of two groups of patients significantly improved compared with before treatment,with statistically significant differences( P <0. 05). The TC levels decreased more significantly. The remaining indicators including TG,LDL-C and HDL-C decreased significantly,with statistical differences( P <0. 05). The incidence of adverse reactions in the two groups were compared,there was significant statistical difference,including gastrointestinal symptoms and abnormal liver function( P <0. 05),while there was no significant statistical difference in acute coronary artery lesions( P >0. 05). Conclusion Inflammation and blood lipid effect of two different types of statins in patients with unstable angina pectoris on reducing postoperative are rela-tively strong,although the adverse reaction in different degree,the occurrence rate is still relatively low,in the controllable range. Therefore,the two kinds of statins is safe and reliable.
