目的：探讨乌司他丁对呼吸机相关性肺损伤老年患者细胞因子及生活质量的影响。方法选取呼吸机相关性肺损伤老年患者90例，以随机数字表法分为对照组（45例）和乌司他丁组（45例）；对照组患者给予常规对症支持治疗；乌司他丁组患者则在此基础上加用乌司他丁微量泵入治疗；比较两组患者治疗前后血清细胞因子水平和圣乔治呼吸问卷评分等。结果对照组患者治疗后TNF-α，IL-6及IL-8指标分别为（283.61±78.13）、（135.16±58.33）、（103.81±56.13）pg/mL；乌司他丁组患者治疗后TNF-α，IL-6及IL-8指标分别为（159.24±52.47）、（73.49±30.67）、（73.14±48.80）pg/mL；两组患者治疗后血清细胞因子水平和圣乔治呼吸问卷评分均显著优于治疗前，且乌司他丁组患者治疗后改善程度优于对照组(P<0.05)。结论乌司他丁对呼吸机相关性肺损伤老年患者可有效减低机体炎症水平，提高生活质量。

목적：탐토오사타정대호흡궤상관성폐손상노년환자세포인자급생활질량적영향。방법선취호흡궤상관성폐손상노년환자90례，이수궤수자표법분위대조조（45례）화오사타정조（45례）；대조조환자급여상규대증지지치료；오사타정조환자칙재차기출상가용오사타정미량빙입치료；비교량조환자치료전후혈청세포인자수평화골교치호흡문권평분등。결과대조조환자치료후TNF-α，IL-6급IL-8지표분별위（283.61±78.13）、（135.16±58.33）、（103.81±56.13）pg/mL；오사타정조환자치료후TNF-α，IL-6급IL-8지표분별위（159.24±52.47）、（73.49±30.67）、（73.14±48.80）pg/mL；량조환자치료후혈청세포인자수평화골교치호흡문권평분균현저우우치료전，차오사타정조환자치료후개선정도우우대조조(P<0.05)。결론오사타정대호흡궤상관성폐손상노년환자가유효감저궤체염증수평，제고생활질량。
Objective To investigate the effects of ulinastatin on cell factor and quality of life of elderly patients with ventilator in-duced lung injury. Methods 90 elderly patients with ventilator induced lung injury were chosen and randomly divided into both group including control group (45 patients) with conventional symptomatic treatment and ulinastatin group (45 patients) with ulinas-tatin treatment on the basis of control group; and the serum levels of cytokines and scores of St. George's Respiratory Question-naire of both groups were compared. Results The serum levels of TNF-α, IL-6 and IL-8 of control group after treatment were separately (283.61±78.13)、(135.16±58.33)、(103.81±56.13)pg/mL; The serum levels of TNF-α, IL-6 and IL-8 of ulinastatin group after treatment were separately (159.24 ±52.47)、(73.49 ±30.67)、(73.14 ±48.80)pg/mL;The serum levels of cytokines and scores of St. George's Respiratory Questionnaire after treatment of both groups was significantly better than before treatment(P<0.05). The serum levels of cytokines and scores of St. George's Respiratory Questionnaire after treatment of ulinastatin group was signifi-cantly better than control group(P<0.05). Conclusion Ulinastatin in treatment of elderly patients with ventilator induced lung in-jury can efficiently reduce inflammatory level and improve quality of life.
