目的：探讨无创呼吸机治疗COPD呼吸衰竭及慢性心功能不全的临床疗效。方法：2011年1月-2013年10月收治COPD、呼吸衰竭合并有慢性心功能不全患者68例，随机分为治疗组和对照组，各34例，对照组给予常规一般治疗，治疗组在对照组的基础上配合无创呼吸机治疗，治疗48 h后比较两组患者的心功能、心率、血气分析、左室射血分数等。结果：治疗后，治疗组 PaO2、SpO2、pH 值、PO2/FiO2、LVEF 明显升高，PaCO2、HR、RR 明显下降，与对照组相比，差异有统计学意义(P＜0.05)。结论：BiPAP呼吸机治疗慢性心力衰竭患者，可以改善患者心功能及临床症状，值得临床推广应用。
목적：탐토무창호흡궤치료COPD호흡쇠갈급만성심공능불전적림상료효。방법：2011년1월-2013년10월수치COPD、호흡쇠갈합병유만성심공능불전환자68례，수궤분위치료조화대조조，각34례，대조조급여상규일반치료，치료조재대조조적기출상배합무창호흡궤치료，치료48 h후비교량조환자적심공능、심솔、혈기분석、좌실사혈분수등。결과：치료후，치료조 PaO2、SpO2、pH 치、PO2/FiO2、LVEF 명현승고，PaCO2、HR、RR 명현하강，여대조조상비，차이유통계학의의(P＜0.05)。결론：BiPAP호흡궤치료만성심력쇠갈환자，가이개선환자심공능급림상증상，치득림상추엄응용。
Objective:To investigate the clinical efficacy of noninvasive ventilation in the treatment of COPD with respiratory failure and chronic heart failure.Methods:68 patients with COPD,respiratory failure with chronic heart failure were selected from January 2011 to October 2013.They were randomly divided into the treatment group and the control group with 34 cases in each. The control group were given the conventional treatment.The treatment group were given noninvasive ventilator therapy based on the control group.After 48 h treatment,we compared the heart function,heart rate,blood gas analysis,left ventricular ejection fraction between two groups.Results:After the treatment,the PaO2,SpO2,pH,PO2/FiO2,LVEF of the patients in the treatment group were increased significantly;the PaCO2,HR,RR were decreased significantly.Compared with the control group,the difference was statistically significant(P<0.05).Conclusion:BiPAP ventilation in the treatment of patients with chronic heart failure can improve cardiac function and clinical symptoms,so it is worthy of clinical application.
