目的:探讨肾性高血压患者血浆网膜素-1水平与血压、胰岛素抵抗和颈动脉粥样硬化(AS)的关系。方法选择肾性高血压患者83例(非斑块组35例,斑块组45例)作为患者组,健康体检者35例作为对照组,应用 ELISA 方法测定受检者血浆网膜素-1水平及相关生化指标,使用超声诊断仪测量颈动脉内膜中层厚度(IMT)。结果血浆网膜素-1水平斑块组和非斑块组均低于对照组(P 均<0.05);斑块组血浆网膜素-1水平显著低于非斑块组(P <0.05)。 Pearson 相关分析显示:血浆网膜素-1水平与收缩压、舒张压、高敏 C 反应蛋白、肌酐、视黄醇结合蛋白、颈动脉 IMT 和胰岛素抵抗指数均呈负相关(r =-0.355,-0.388,-0.125,-0.491,-0.166,-0.721和-0.539,P 均<0.05);多元逐步 Logistic 回归分析结果显示颈动脉 IMT、胰岛素抵抗指数、收缩压和低密度脂蛋白是网膜素-1的独立影响因素。结论血浆网膜素-1水平随血压升高而降低,胰岛素抵抗和颈动脉 IMT 与网膜素-1变化关系密切,低血浆网膜素-1导致肾性高血压的发生发展及颈动脉 IMT 增厚,为防止肾性高血压及预防心血管危险因素找到有效药物作用途径。

목적:탐토신성고혈압환자혈장망막소-1수평여혈압、이도소저항화경동맥죽양경화(AS)적관계。방법선택신성고혈압환자83례(비반괴조35례,반괴조45례)작위환자조,건강체검자35례작위대조조,응용 ELISA 방법측정수검자혈장망막소-1수평급상관생화지표,사용초성진단의측량경동맥내막중층후도(IMT)。결과혈장망막소-1수평반괴조화비반괴조균저우대조조(P 균<0.05);반괴조혈장망막소-1수평현저저우비반괴조(P <0.05)。 Pearson 상관분석현시:혈장망막소-1수평여수축압、서장압、고민 C 반응단백、기항、시황순결합단백、경동맥 IMT 화이도소저항지수균정부상관(r =-0.355,-0.388,-0.125,-0.491,-0.166,-0.721화-0.539,P 균<0.05);다원축보 Logistic 회귀분석결과현시경동맥 IMT、이도소저항지수、수축압화저밀도지단백시망막소-1적독립영향인소。결론혈장망막소-1수평수혈압승고이강저,이도소저항화경동맥 IMT 여망막소-1변화관계밀절,저혈장망막소-1도치신성고혈압적발생발전급경동맥 IMT 증후,위방지신성고혈압급예방심혈관위험인소조도유효약물작용도경。
Objective It is to investigate the relationship between plasma omentin -1 level and blood pressure, insulin re-sistance and carotid artery atherosclerosis (AS)in patients with renal hypertention.Methods 83 cases of renal hypertensive patients (45 cases of plaques, 35 cases of no plaques) as patient group and 35 healthy subjects as control group were enrolled into this study, Plama omentin -1 level and correlated biochemical indicators were measured by ELISA,carotid intima-media thickness(IMT) was estimated by ultrasonic diagnostic equipment.Results The plasma levels of omentin -1 of the plaques group and no plaques group were lower than that of control group(P <0.05); Plasma omentin -1 level in the plaques group was significantly lower than that of the no plaques group(P <0.05).Pearson correlation analysis revealed that omentin -1 was negative correlated with systolic blood pressure(SBP),diastolic pressure(DBP),high sensitive C -reactive protein,creatinine, retinol binding protein,carotid IMT and insulin resistance index (r =-0.355, -0.388, -0.125, -0.491, -0.166, -0.721 and -0.539,P <0.05); Multiple stepwise logistic regression analysis indicated that carotid artery IMT, insulin resist-ance index,SBP and low density lipoprotein(LDL) were independent factors associated with plasma omentin -1 level(P <0.05).Conclusion The level of omentin -1 decreased in the patients with renal hypertention with increased blood pressure, insulin resistance and carotid IMT closely associated with changes in omentin -1, low plasma omentin -1 could lead to the de-velopment of the occurrence of renal hypertension and the formation of carotid IMT, in order to prevent renal hypertensive car-diovascular risk factors for finding a way to effective drugs.
