目的:探讨吸烟慢性阻塞性肺疾病(COPD)稳定期患者、健康吸烟者及健康非吸烟者外周血中白介素(IL)-18及 Treg 细胞变化及其临床意义。方法选择2012年11月—2013年5月于河北省人民医院呼吸内科门诊就诊的吸烟 COPD 患者33例为吸烟 COPD 组,体检中心体检健康者分为健康吸烟组19例和健康非吸烟组16例。采用酶联免疫吸附测定(ELISA)法检测受试者外周血中IL -18、IL -17水平,用流式细胞术检测 CD8+及 Treg 细胞比例。结果吸烟 COPD 组IL -18、IL -17、CD8+较健康吸烟组、健康非吸烟组均升高,Treg 细胞较健康吸烟组下降、较健康非吸烟组升高(P <0,05);健康吸烟组IL -18、IL -17、CD8+及 Treg 细胞较健康非吸烟组均升高( P <0,05)。结论 IL -18分泌增多,诱导IL -17、CD8+细胞上升,超出 Treg 细胞的抑制能力以及 Treg 细胞的反应钝化可能导致吸烟COPD 患者气道的慢性炎症。

목적:탐토흡연만성조새성폐질병(COPD)은정기환자、건강흡연자급건강비흡연자외주혈중백개소(IL)-18급 Treg 세포변화급기림상의의。방법선택2012년11월—2013년5월우하북성인민의원호흡내과문진취진적흡연 COPD 환자33례위흡연 COPD 조,체검중심체검건강자분위건강흡연조19례화건강비흡연조16례。채용매련면역흡부측정(ELISA)법검측수시자외주혈중IL -18、IL -17수평,용류식세포술검측 CD8+급 Treg 세포비례。결과흡연 COPD 조IL -18、IL -17、CD8+교건강흡연조、건강비흡연조균승고,Treg 세포교건강흡연조하강、교건강비흡연조승고(P <0,05);건강흡연조IL -18、IL -17、CD8+급 Treg 세포교건강비흡연조균승고( P <0,05)。결론 IL -18분비증다,유도IL -17、CD8+세포상승,초출 Treg 세포적억제능력이급 Treg 세포적반응둔화가능도치흡연COPD 환자기도적만성염증。
Objective To explore changes and clinical significances of IL - 18 and Treg in peripheral blood of patients with stable chronic obstructive pulmonary disease(COPD),healthy smokers and healthy control non - smokers, Methods 33 smoker patients with COPD(COPD group),19 healthy smokers(healthy smoker group)and 16 healthy non - smokers(healthy non - smoker group)admitted to the Department of Respiratory Medicine of Hebei General Hospital from November 2012 to May 2013 were selected, ELISA method was used to determine IL - 18 and IL - 17 in peripheral blood,and flow cytometry method was used to determine CD8 + and Treg, Results The IL - 18,IL - 17 and CD8 + in COPD group was higher than the healthy smoker group and healthy non - smoker group,and Treg in COPD group was lower than the healthy smoker group,but higher than the healthy non - smoker group(P < 0, 05), The IL - 18,IL - 17,CD8 + and Treg were higher in healthy smoker group than the healthy non - smoker group(P < 0, 05), Conclusion Patients with COPD have an increase in IL - 18,IL - 17,CD8 + cells with a blunted Treg cells response,resulting in chronic airway inflammation.
