目的：探讨肝癌切除术中植入5-氟尿嘧啶（5-Fu）缓释剂进行区域性化疗的安全性及临床疗效。方法选择2011－2012年于新疆医科大学第一附属医院腔镜肝移植外科就诊的可经手术探查并能完整切除的肝癌患者54例，随机分为5-Fu 缓释剂残肝断面植入组（植入组，26例）及对照组（28例）。所有患者分别于入院第1天和术后第14天采血，术后2 w 观察并记录两组患者的不良反应、血常规、肝肾功能及肿瘤标记物水平，并对其观察结果进行统计学分析。结果（1）术后2 w 植入组与对照组患者白细胞计数及肾功能分级比较差异无统计学意义（P ＞0．05）；（2）术后2 w 植入组与对照组患者不良反应（恶心呕吐、发热分度、脱发分度）比较差异无统计学意义（P ＞0．05）；（3）植入组与对照组患者术后切口愈合等级比较差异无统计学意义（P ＞0．05）；（4）术后2 w 植入组与对照组患者 ALT、AST、TBL 及 PT 比较差异无统计学意义（P ＞0．05），AFP 比较差异具有统计学意义（P ＜0．05）。结论肝癌切除术中植入5-Fu 缓释剂短期内无明显药物毒副作用，对血常规及肝肾功能无显著影响，能显著降低 AFP 水平，是一种安全可靠有效的局部的化疗药物。
목적：탐토간암절제술중식입5-불뇨밀정（5-Fu）완석제진행구역성화료적안전성급림상료효。방법선택2011－2012년우신강의과대학제일부속의원강경간이식외과취진적가경수술탐사병능완정절제적간암환자54례，수궤분위5-Fu 완석제잔간단면식입조（식입조，26례）급대조조（28례）。소유환자분별우입원제1천화술후제14천채혈，술후2 w 관찰병기록량조환자적불량반응、혈상규、간신공능급종류표기물수평，병대기관찰결과진행통계학분석。결과（1）술후2 w 식입조여대조조환자백세포계수급신공능분급비교차이무통계학의의（P ＞0．05）；（2）술후2 w 식입조여대조조환자불량반응（악심구토、발열분도、탈발분도）비교차이무통계학의의（P ＞0．05）；（3）식입조여대조조환자술후절구유합등급비교차이무통계학의의（P ＞0．05）；（4）술후2 w 식입조여대조조환자 ALT、AST、TBL 급 PT 비교차이무통계학의의（P ＞0．05），AFP 비교차이구유통계학의의（P ＜0．05）。결론간암절제술중식입5-Fu 완석제단기내무명현약물독부작용，대혈상규급간신공능무현저영향，능현저강저 AFP 수평，시일충안전가고유효적국부적화료약물。
Objective To Investigate the release of liver cancer during implantation for regional chemothera-py agent fluorouracil safety and clinical efficacy.Methods 54 cases of liver cancer patients who can suf-fered surgical exploration and be cut completely were Collected from 2011 to 2012,and were randomized into treatment group and control group.After surgery,patients in treatment group were implanted with 5-fluorouracil into the incisal edge of liver while patients in control group without any chemotherapy.Draw-blood were taken to all patients on the 1st day after admission to hospital and 14th day after surgery.Ad-verse reaction,routine blood test,liver function,renal function,tumor maker were observed and com-pared between the two groups two weeks after surgery.Results (1)There is no significant different be-tween the two group of adverse reaction,routine blood test,liver function and renal function (P >0.05). (2)There is significant different between the two group of AFP two weeks after surgery (P <0.05).Con-clusion Implanted with 5-fluorouracil into the incisal edge of liver while surgery has no significant adverse reaction and harmful effects to routine blood test,liver function and renal function,and can reduce AFP significantly.It can be recognized as a Safe and useful partial chemotherapy medicine.
