目的:探讨早期肠内营养对大鼠胃缺血-再灌注损伤(gastric ischemia-reperfusion inj ury,GI-RI)的影响及主要作用机制。方法建立大鼠GI-RI模型,将实验动物采用随机数字表法分为早期肠内营养组(early enteral nutrition,EEN)、单纯损伤组(I)和正常对照组(C),肉眼计数胃黏膜损伤指数、光镜下行病理评分、采用比色法检测胃黏膜脂质过氧化物丙二醛(male-ic dialdehvde,MDA)含量和血清超氧化物歧化酶(superoxide dismutase,SOD)活力,并观察大鼠的体质量变化。结果 EEN组和 I组的胃黏膜溃疡指数、病理评分、MDA含量、体质量下降值在 GI-RI后明显高于对照组(P<0.05),SOD活力明显低于对照组(P<0.05)。EEN组和 I组比较,除6 h以外的各时间点,EEN组胃黏膜损伤指数、MDA含量、体质量下降值均明显低于I组(P<0.05),SOD活力高于I组(P<0.01)。72 h病理评分EEN组低于I组(P<0.05)。结论早期肠内营养能明显减轻大鼠缺血-再灌注后的胃黏膜损伤,其可能机制是通过促进机体氧化与抗氧化平衡实现的。

목적:탐토조기장내영양대대서위결혈-재관주손상(gastric ischemia-reperfusion inj ury,GI-RI)적영향급주요작용궤제。방법건립대서GI-RI모형,장실험동물채용수궤수자표법분위조기장내영양조(early enteral nutrition,EEN)、단순손상조(I)화정상대조조(C),육안계수위점막손상지수、광경하행병리평분、채용비색법검측위점막지질과양화물병이철(male-ic dialdehvde,MDA)함량화혈청초양화물기화매(superoxide dismutase,SOD)활력,병관찰대서적체질량변화。결과 EEN조화 I조적위점막궤양지수、병리평분、MDA함량、체질량하강치재 GI-RI후명현고우대조조(P<0.05),SOD활력명현저우대조조(P<0.05)。EEN조화 I조비교,제6 h이외적각시간점,EEN조위점막손상지수、MDA함량、체질량하강치균명현저우I조(P<0.05),SOD활력고우I조(P<0.01)。72 h병리평분EEN조저우I조(P<0.05)。결론조기장내영양능명현감경대서결혈-재관주후적위점막손상,기가능궤제시통과촉진궤체양화여항양화평형실현적。
Objective To investigate the effects of early enteral nutrition on rats gastric ischemia reperfu-sion inj ury.Methods Setting GI-RI mode,and the rats were randomly divided into 3 groups:early enteral nutrition group(EEN group),injury group(I group),and control group(C group).Gastric mucosal injury index,light microscope pathological score,gastric mucous membrane lipid peroxide MDA and vitality of serum superoxide dismutase was tested by using colorimetric method,and body mass change were observed.Results The gastric mucosal inj ury index,pathological score,the content of MDA,the value of weight loss in both EEN group and inj ury group were higher than those in control group after GI-RI (P<0.05).Meanwhile,the activity of SOD were lower than control group (P<0.05).The gastric mucosal injury index,the content of MDA,the value of weight loss exhibited significant decrease in EEN group compared with injury group at all time points except 6 h after the reperfusion (P<0.05),and the activity of SOD was higher than injury group(P<0.01).The pathological score was lower in EEN group than injury group at 72 h after reperfusion (P<0.05 ).Conclusion This study shows that early enteral nutrition exhibits a gastro protective effect against acute gastric mucosal lesions in rats induced by ischemia-reperfusion,which probably involves promoting the balance of peroxidation and antioxidation.
