Closed poking reduction of calcaneal fractures%Open reduction
目的：回顾性分析切开复位钢板螺钉内固定、C型臂透视下经皮撬拨闭合复位内固定治疗SandersⅡ型以上跟骨关节内骨折的临床资料，比较研究2种手术固定方法治疗跟骨关节内骨折的临床疗效。方法：选取自2008年12月1日～2013年6月1日，我院共收治的SandersⅡ型以上的跟骨关节内骨折患者19例，将其分为对照组与治疗组。对照组9例，治疗组10例，分别采用骨圆针撬拨为主结合手法复位石膏管型外固定治疗与切开复位内固定治疗。随访6～12个月，平均9个月，对2种治疗结果进行统计学处理与对比扥系，评价2种治疗方案的临床效果。结果：依据Maryland足评分对2种治疗方案进行评价，评价结果为：（1）在恢复Gissane角、Bolher角和纠正跟骨增宽方面，2种治疗方案均产生了较好作用（P＜0．05）；（2）对2种方案治疗的综合疗效进行评价，治疗组优良率达到90％，对照组优良率为77．8％，2组有极显著性差异（ P＜0．01），其治疗组明显优于对照组。结论：经过2种治疗方案临床效果的比较研究，对于SandersⅡ型以上跟骨关节内骨折的患者，采用切开复位钢板内固定法的治疗效果明显优于透视下经皮闭合撬拨复位内固定。

목적：회고성분석절개복위강판라정내고정、C형비투시하경피효발폐합복위내고정치료SandersⅡ형이상근골관절내골절적림상자료，비교연구2충수술고정방법치료근골관절내골절적림상료효。방법：선취자2008년12월1일～2013년6월1일，아원공수치적SandersⅡ형이상적근골관절내골절환자19례，장기분위대조조여치료조。대조조9례，치료조10례，분별채용골원침효발위주결합수법복위석고관형외고정치료여절개복위내고정치료。수방6～12개월，평균9개월，대2충치료결과진행통계학처리여대비돈계，평개2충치료방안적림상효과。결과：의거Maryland족평분대2충치료방안진행평개，평개결과위：（1）재회복Gissane각、Bolher각화규정근골증관방면，2충치료방안균산생료교호작용（P＜0．05）；（2）대2충방안치료적종합료효진행평개，치료조우량솔체도90％，대조조우량솔위77．8％，2조유겁현저성차이（ P＜0．01），기치료조명현우우대조조。결론：경과2충치료방안림상효과적비교연구，대우SandersⅡ형이상근골관절내골절적환자，채용절개복위강판내고정법적치료효과명현우우투시하경피폐합효발복위내고정。
Objective:To analyze retrospectively of open reduction and internal fixation with plate and screw, C arm fluoroscopy guided percutaneous poking clinical data of closed reduction and internal fixation of Sanders type II or above of intra-articular calcaneal fracture, treatment of clinical efficacy of intra-articular calcaneal fracture of two methods for fixed operation.Methods:from December 1, 2008 to June 1, 2013, the SandersⅡ were treated in our hospital of intra-articular calcaneal fractures in 19 patients, divided into treatment group and control group.9 cases of the control group, the treatment group of 10 cases, respectively, using bone round needle poking com-bined with manual reduction and plaster cast external fixation and open reduction and internal fixation.Patients were followed up for 6 ~12 months, average 9 months, were analyzed statistically and compared to create lines to two treatment results, from the clinical effect of two therapeutic schemes evaluation.Results:According to the Maryland foot score evaluation of two therapeutic schemes, the evaluation re-sults were:(1) on the recovery of Gissane angle, Bolher angle and correcting calcaneus broadening, two types of therapy had better effect ( P<0.05);(2) evaluate the comprehensive curative effect in treatment of two schemes, treatment excellent rate reached 90%, the con-trol group was 77.8%excellent, two groups had significant difference (P<0.01), the treatment group was significantly better than the control group.Conclusion:Through the comparative study of clinical effect of two kinds of treatment, the Sanders Ⅱ of intra-articular calcaneal fracture patients, the treatment effect should be better than open reduction and internal fixation method under fluoroscopic percu-taneous closure prying reduction and internal fixation.
