目的:探讨定量组织速度成像(quantitative tissue velocity imaging,QTVI)及组织追踪成像(tissue tracing imaging,TTI)对矽肺患者右室收缩功能变化的评估价值。方法:选择2012年11月—2014 年 1月江苏省南京市胸科医院收治的矽肺患者42例(矽肺组);同时选择性别、年龄相匹配的正常成人36例作为对照组。两组均行二维超声心动图检查,测量右心腔内径、右心室面积变化率(right ventricular fractional area change,RVFAC)、肺动脉收缩压(pulmonary artery systolic pressure, PASP)。应用 QTVI 获取右室游离壁三尖瓣环收缩期峰值速度(tricuspid annular peak systolic velocity,TAPVs);应用 TTI 获取右室游离壁三尖瓣环收缩期位移(tricuspid annular plane systolic excursion,TAPSE)。结果:矽肺组患者右心腔内径、RV-FAC、PASP 与对照组差异均无统计学意义(P >0.05);矽肺组 TAPVs、TAPSE 均较对照组减低(P <0.01)。结论:QTVI 及TTI 可以定量、简便、敏感地评价矽肺患者右室收缩功能的变化。

목적:탐토정량조직속도성상(quantitative tissue velocity imaging,QTVI)급조직추종성상(tissue tracing imaging,TTI)대석폐환자우실수축공능변화적평고개치。방법:선택2012년11월—2014 년 1월강소성남경시흉과의원수치적석폐환자42례(석폐조);동시선택성별、년령상필배적정상성인36례작위대조조。량조균행이유초성심동도검사,측량우심강내경、우심실면적변화솔(right ventricular fractional area change,RVFAC)、폐동맥수축압(pulmonary artery systolic pressure, PASP)。응용 QTVI 획취우실유리벽삼첨판배수축기봉치속도(tricuspid annular peak systolic velocity,TAPVs);응용 TTI 획취우실유리벽삼첨판배수축기위이(tricuspid annular plane systolic excursion,TAPSE)。결과:석폐조환자우심강내경、RV-FAC、PASP 여대조조차이균무통계학의의(P >0.05);석폐조 TAPVs、TAPSE 균교대조조감저(P <0.01)。결론:QTVI 급TTI 가이정량、간편、민감지평개석폐환자우실수축공능적변화。
Objective:To evaluate the change of the right ventricular systolic function in patients with silicosis by quantitative tissue velocity imaging(QTVI)and tissue tracing imaging(TTI).Methods:A total of 42 patients with silicosis treated in Nan-jing Chest Hospital from Nov 2012 to Jan 2014 (silicosis group)and 36 normal controls (control group)were enrolled in this study.The right heart diameter,right ventricular fractional area change (RVFAC)and pulmonary artery systolic pressure (PASP)were studied by two dimensional echocardiogram(2DE).The tricuspid annular peak systolic velocity(TAPVs)and tri-cuspid annular plane systolic excursion(TAPSE)were measured by QTVI and TTI.Results:There were no significant differ-ence in the right heart diameter,RVFAC and PASP between the two groups.And the TAPVs,TAPSE in the silicosis group were lower than those in the control group (P <0.01).Conclusions:QTVI and TTI may provide a quantitative,simple and sen-sitive tool for evaluation of the right ventricular systolic function in patients with silicosis.
