目的:观察持续被动运动(CPM)对偏瘫患者股四头肌张力及下肢运动功能的影响。方法选取存在下肢痉挛的偏瘫患者50例,将其随机分为治疗组和对照组,每组各25例。对照组采用常规康复治疗,治疗组在对照组基础上结合下肢CPM治疗,比较两组治疗后4、6周的效果。结果两组治疗后6周的股四头肌张力评分均有所下降,与治疗前比较,差异有统计学意义(P<0.01),治疗组治疗后6周的股四头肌张力评分显著低于对照组(P<0.05)。两组治疗后4、6周的FMA下肢运动功能评分均显著提高,与治疗前比较,差异有统计学意义(P<0.05)。治疗组治疗后4、6周的FMA评分高于对照组,但差异无统计学意义(P>0.05)。结论 CPM能降低偏瘫患者股四头肌张力以及改善患者的下肢运动功能。

목적:관찰지속피동운동(CPM)대편탄환자고사두기장력급하지운동공능적영향。방법선취존재하지경련적편탄환자50례,장기수궤분위치료조화대조조,매조각25례。대조조채용상규강복치료,치료조재대조조기출상결합하지CPM치료,비교량조치료후4、6주적효과。결과량조치료후6주적고사두기장력평분균유소하강,여치료전비교,차이유통계학의의(P<0.01),치료조치료후6주적고사두기장력평분현저저우대조조(P<0.05)。량조치료후4、6주적FMA하지운동공능평분균현저제고,여치료전비교,차이유통계학의의(P<0.05)。치료조치료후4、6주적FMA평분고우대조조,단차이무통계학의의(P>0.05)。결론 CPM능강저편탄환자고사두기장력이급개선환자적하지운동공능。
Objective To observe the influence of continuous passive motion (CPM) on quadriceps tension and the low-er limb movement function in patients with stroke hemiplegia. Methods 50 patients with hemiplegia having lower limb spastic were selected,and they were randomly divided into the treatment group (n=25) and control group (n=25),the control group was given the routine rehabilitation treatment,the treatment group was given CPM on the basis of the con-trol group,and then the curative effect after 4,6 weeks treatment were compared. Results The quadriceps tension score after 6 weeks treatment in the two groups decreased compared with before treatment,and there was a statistical differ-ence (P<0.01),the quadriceps tension score after 6 weeks treatment in the treatment group was obviously lower than that in the control group (P<0.05).The FMA lower limb motor function score in the two groups after 4,6 weeks treatment in-creased compared with before treatment,and there was a statistical difference (P<0.05), the FMA lower limb motor func-tion score after 4,6 weeks treatment in the treatment group were higher than those in the control group,but there was no statistical difference (P>0.05). Conclusion CPM can reduce the quadriceps tension and improve the function of lower limb in patients with hemiplegia.
