目的：探讨不同术式联合矫正老年性白内障患者术前散光的效果和可行性。方法选取2013年1月~2014年6月本院收治的102例(102眼)老年性白内障散光患者作为研究对象,随机分为对照组(51眼)和观察组(51眼),对照组行球面人工晶状体联合角膜缘松解术,观察组行超声乳化白内障摘除联合散光型人工晶状体(Toric IOL)植入术,随访3个月,比较两组裸眼视力、最佳矫正视力、柱镜度数和散光矫正率的差异。结果两组的最佳矫正视力比较,差异无统计学意义(P>0.05)；观察组的裸眼视力、柱镜度数显著优于对照组,差异有统计学意义(P<0.05)；观察组的散光矫正率为94.37%,显著高于对照组的63.38%,差异有统计学意义(P<0.05)。结论 Toric-IOL和角膜缘松解术两种术式均是矫正老年性白内障患者术前散光的有效方法,Toric-IOL疗效更稳定,可保证术后最佳裸眼视力和视觉质量。
목적：탐토불동술식연합교정노년성백내장환자술전산광적효과화가행성。방법선취2013년1월~2014년6월본원수치적102례(102안)노년성백내장산광환자작위연구대상,수궤분위대조조(51안)화관찰조(51안),대조조행구면인공정상체연합각막연송해술,관찰조행초성유화백내장적제연합산광형인공정상체(Toric IOL)식입술,수방3개월,비교량조라안시력、최가교정시력、주경도수화산광교정솔적차이。결과량조적최가교정시력비교,차이무통계학의의(P>0.05)；관찰조적라안시력、주경도수현저우우대조조,차이유통계학의의(P<0.05)；관찰조적산광교정솔위94.37%,현저고우대조조적63.38%,차이유통계학의의(P<0.05)。결론 Toric-IOL화각막연송해술량충술식균시교정노년성백내장환자술전산광적유효방법,Toric-IOL료효경은정,가보증술후최가라안시력화시각질량。
Objective To explore the clinical effect and feasibility of different surgical combined with correction for se-nile cataract patients with preoperative astigmatism. Methods 102 cases (102 eyes) of senile cataract patients with pre-operative astigmatism in our hospital from January 2013 to May 2014 were randomly divided into control group(51 eyes) and observation group(51 eyes).The control group were treated with intraocular lens combined with corneal limbal lysis. The observation group were treated with phacoemulsification cataraction combined with Toric IOL.All patients were fol-lowed up for 3 months.The uncorrected visual acuity,best corrected visual acuity,columns diopters and astigmatism cor-rection rate of two groups were compared. Results The best corrected visual acuity of two groups had no significant dif-ference (P>0.05).The uncorrected visual acuity and columns diopters of observation group were significantly better than that of control group (P<0.05).The astigmatism correction rate of observation group was 94.37%,which was significantly higher than 63.38%of control group(P<0.05). Conclusion Toric IOL and corneal limbal lysis are both effective surgical methods to correct senile cataract with properative astigmatism.Toric IOL is more stable,it can ensure the best postoper-ative visual acuity and visual quality.
