目的:探讨PDCA循环法在控制重症监护病房(ICU)有创呼吸机使用患者多重耐药菌中的应用效果。方法回顾性地调查分析2011年本院ICU有创呼吸机使用患者多重耐药菌感染发生的原因,2012年开始将计划、实施、检查、处理(PDCA)循环法引入到ICU有创呼吸机使用患者多重耐药菌感染控制中,通过目标性监测,发现ICU有创呼吸机使用患者多重耐药菌感染的隐患,针对性制订控制感染的干预措施及操作要求,落实干预措施并加以巩固,观察2011、2012年有创呼吸机使用患者多重耐药菌的检出率和类别的变化。结果运用PDCA循环法后,ICU有创呼吸机使用患者中多重耐药菌的检出率明显降低(P<0.01)。结论ICU引进PDCA循环法能有效降低有创呼吸机使用患者多重耐药菌发生率,减少院内感染,提高医疗质量、保障患者的安全、降低患者的医疗费用,值得运用推广。

목적:탐토PDCA순배법재공제중증감호병방(ICU)유창호흡궤사용환자다중내약균중적응용효과。방법회고성지조사분석2011년본원ICU유창호흡궤사용환자다중내약균감염발생적원인,2012년개시장계화、실시、검사、처리(PDCA)순배법인입도ICU유창호흡궤사용환자다중내약균감염공제중,통과목표성감측,발현ICU유창호흡궤사용환자다중내약균감염적은환,침대성제정공제감염적간예조시급조작요구,락실간예조시병가이공고,관찰2011、2012년유창호흡궤사용환자다중내약균적검출솔화유별적변화。결과운용PDCA순배법후,ICU유창호흡궤사용환자중다중내약균적검출솔명현강저(P<0.01)。결론ICU인진PDCA순배법능유효강저유창호흡궤사용환자다중내약균발생솔,감소원내감염,제고의료질량、보장환자적안전、강저환자적의료비용,치득운용추엄。
Objective To investigate the application effect of plan-do-check action (PDCA) cycle in the control of multiple drug-resistant bacteria (MDRB) in patients with invasive ventilator in intensive care unit (ICU). Methods Causes of MDRB occurrence in patients with invasive ventilator in ICU in 2011 were retrospectively surveyed and an-alyzed.At the beginning of 2012,PDCA cycle was introduced into the control of MDRB in patients with invasive ventila-tor in ICU.By objective monitoring,infected hidden danger of MDRB in patients with invasive ventilator in ICU was de-tected.After formulating targeted intervention measures of controlling infection and operation requirements and imple-menting and strengthening these measures,the detection rate and variation of category of MDRB in patients with inva-sive ventilator in 2011 and 2012 were observed. Results After PDCA cycle being applied,the detection rate of MDRB in patients with invasive ventilator in ICU greatly decreased (P<0.01). Conclusion PDCA cycle introduced into ICU can effectively decrease the incidence rate of MDRB in patients with invasive ventilator, reduce nosocomial infection, improve quality of medical care,protect safety of patients and lower down medical costs of patients.It is worthy of appli-cation and expansion.
