目的：研究氯沙坦联合螺内酯片治疗原发性高尿酸血症并高血压的效果及其对三酰甘油(TG)的影响。方法以2013年2月~2014年5月本院收治的80例原发性高尿酸血症并高血压患者为研究对象，按照随机数字表法均分为观察组与对照组，观察组给予氯沙坦联合螺内酯片治疗，对照组仅行氯沙坦治疗。连续用药8周，比较治疗前(T1)，治疗后2、4、6、8周(T2、T3、T4、T5)血压与血尿酸的变化，同时比较血TG的变化。结果两组血尿酸均得到有效控制，其水平随时间延长呈现显著下降趋势，差异有统计学意义(P<0.05)，且从T3时刻开始，数据较T1时刻差异有统计学意义(P<0.05)；两组各时间点血尿酸水平差异无统计学意义(P>0.05)。两组血压均得到有效控制，舒张压及收缩压均随时间延长显著下降(P<0.05)；但各时间点组间比较，差异无统计学意义(P>0.05)。两组血TG水平均呈上升趋势(P<0.05)；自T3开始，观察组血TG相较对照组更低(P<0.05)。结论氯沙坦联合螺内酯片可有效降低原发性高尿酸血症并高血压患者的血尿酸及血压水平，同时还有助于抑制血TG升高。
목적：연구록사탄연합라내지편치료원발성고뇨산혈증병고혈압적효과급기대삼선감유(TG)적영향。방법이2013년2월~2014년5월본원수치적80례원발성고뇨산혈증병고혈압환자위연구대상，안조수궤수자표법균분위관찰조여대조조，관찰조급여록사탄연합라내지편치료，대조조부행록사탄치료。련속용약8주，비교치료전(T1)，치료후2、4、6、8주(T2、T3、T4、T5)혈압여혈뇨산적변화，동시비교혈TG적변화。결과량조혈뇨산균득도유효공제，기수평수시간연장정현현저하강추세，차이유통계학의의(P<0.05)，차종T3시각개시，수거교T1시각차이유통계학의의(P<0.05)；량조각시간점혈뇨산수평차이무통계학의의(P>0.05)。량조혈압균득도유효공제，서장압급수축압균수시간연장현저하강(P<0.05)；단각시간점조간비교，차이무통계학의의(P>0.05)。량조혈TG수평균정상승추세(P<0.05)；자T3개시，관찰조혈TG상교대조조경저(P<0.05)。결론록사탄연합라내지편가유효강저원발성고뇨산혈증병고혈압환자적혈뇨산급혈압수평，동시환유조우억제혈TG승고。
Objective To study the effect of losartan combined with spironolactone tablets in treatment of primary hy-peruricemia and hypertension and the influence on triglycerides (TG). Methods 80 cases of patients with primary hype-ruricemia and hypertension treated in our hospital from February 2013 to May 2014 were selected,and were divided in-to observation group and control group according to the random number table,the observation group were taken with losartan combined with spironolactone tablets for treatment,and the control group were only given with losartan for treatment.Continuous medication for eight weeks,the changes of blood pressure,serum uric acid and TG before treatment (T1) and after 2,4,6,8 weeks treatment (T2,T3,T4,T5) were compared. Results The serum uric acid of two groups were ef-fectively controlled,the level showed a significant downward trend with time,the difference was significant (P<0.05),and from the beginning of time T3,the data had significant difference compared with the time of T1 (P<0.05).The level of serum uric acid of two groups at each time point had no significant difference (P>0.05).The blood pressure of two groups were effectively controlled,diastolic and systolic blood pressure were significantly decreased with the time (P<0.05);but the two groups at each time point had no significant difference (P>0.05).TG level of the two groups showed an upward trend (P<0.05);from the time of T3,TG of the observation group was lower than that of the control group,the dif-ference was significant (P<0.05). Conclusion Losartan combined with spironolactone tablets can effectively reduce the level of serum uric acid and blood pressure in patients with primary hyperuricemia and hypertension,and also can help to control the increasing of TG.
