目的：探讨急症CT平扫在腹部闭合性创伤诊断中的临床应用价值。方法选取本院2011年7月~2014年7月的260例腹部闭合性创伤行急症CT平扫患者作为研究对象,将CT诊断结果同患者实施手术治疗以及采取其他综合治疗后的诊断结果进行比较。结果260例患者中脾脏损伤54例,肾脏损伤34例,肝脏损伤27例,胰腺损伤8例,肠道损伤以及肠系膜损伤18例,胆道损伤2例,腹膜后血肿7例,未表现出脏器损伤的腹腔内少量积血患者8例。122例见不同程度的腹腔积血、积液。50例患者合并出现肋骨、脊柱及骨盆骨折,12例患者合并出现胸腹壁积气症状。CT平扫诊断的准确度为91.5%,灵敏度为91.1%,特异度为92.1%。结论CT平扫检查方便、快速,可清晰显示腹部闭合性创伤患者腹部脏器受损的位置以及受损程度,为患者的临床治疗提供重要参考依据。
목적：탐토급증CT평소재복부폐합성창상진단중적림상응용개치。방법선취본원2011년7월~2014년7월적260례복부폐합성창상행급증CT평소환자작위연구대상,장CT진단결과동환자실시수술치료이급채취기타종합치료후적진단결과진행비교。결과260례환자중비장손상54례,신장손상34례,간장손상27례,이선손상8례,장도손상이급장계막손상18례,담도손상2례,복막후혈종7례,미표현출장기손상적복강내소량적혈환자8례。122례견불동정도적복강적혈、적액。50례환자합병출현륵골、척주급골분골절,12례환자합병출현흉복벽적기증상。CT평소진단적준학도위91.5%,령민도위91.1%,특이도위92.1%。결론CT평소검사방편、쾌속,가청석현시복부폐합성창상환자복부장기수손적위치이급수손정도,위환자적림상치료제공중요삼고의거。
Objective To explore the clinical application value of CT plain scan in the diagnosis of blunt abdominal trauma(BAT) in emergency. Methods 260 patients with BAT from July 2011 to July 2014 in our hospital were examined by emergency CT plain scan.The diagnostic outcome by CT and results from surgical treatment or other comprehensive therapy were compared. Results Among all these participants,the number of injuries in spleen,kidney,liver,pancreas,in-testine and mesentery,biliary tract was 54,34,27,8,18,and 2 respectively.There were retroperitoneal hematoma in 7 cases and a small amount of hematocele in abdominal cavity without obvious injury in viscera in 8 cases.Hemoperitoneum and seroperitoneum occurred at varying degrees in 122 patients.Combined fractures of rib,spine,and pelvis occurred in 50 patients.12 cases combined with pneumatosis in thoracic wall and abdominal wall.The accuracy rate,sensitivity,and specificity of CT plain scan were 91.5%,91.1%,and 92.1% respectively. Conclusion CT plain scan is convenient and fast,and can clearly locate the position of injured abdominal organ and display the degree of injury in patients with BAT,which provide the important reference for clinical treatment.
