目的:探讨初筛未通过的新生儿42天听力复筛结果是否受母体乙肝病毒感染的影响。方法采用畸变产物耳声发射(DPOAE)测试,对初筛未通过且乙肝病毒标志物阳性产妇所产新生儿进行42天听力复筛。结果本研究331例新生儿中,听力复筛通过289例,复筛通过率87.31%。母血HBeAg阳性组的脐血HBsAg与HBeAg阳性新生儿显著高于母血HBeAg阴性组(P<0.01);同时母血HBeAg阳性组以及母血HBV DNA阳性组的复筛通过率低于阴性组(P<0.05)。结论妊娠母血HBeAg阳性和母血HBV DNA阳性与新生儿宫内感染以及听力复筛不通过存在一定联系,今后工作中应密切关注和定期随访。

목적:탐토초사미통과적신생인42천은력복사결과시부수모체을간병독감염적영향。방법채용기변산물이성발사(DPOAE)측시,대초사미통과차을간병독표지물양성산부소산신생인진행42천은력복사。결과본연구331례신생인중,은력복사통과289례,복사통과솔87.31%。모혈HBeAg양성조적제혈HBsAg여HBeAg양성신생인현저고우모혈HBeAg음성조(P<0.01);동시모혈HBeAg양성조이급모혈HBV DNA양성조적복사통과솔저우음성조(P<0.05)。결론임신모혈HBeAg양성화모혈HBV DNA양성여신생인궁내감염이급은력복사불통과존재일정련계,금후공작중응밀절관주화정기수방。
Objective To explore whether the infective factor will affect the results of 42-days hearing screening in newborns whose mother with hepatitis B. Methods All theinfants born to mothers with hepatitis B performed with 42-days hearing screening in distortion product otoacoustic emissions(DPOAE). Results Among331 cases, 289 cases passed 42-days hearing screening.The rate was 87.31%. In the groups with mother pregnancywithHBeAg(+),the rate of neonates with HBsAg(+) and HBeAg(+) wassignificantly higher than that of neonates whose mother pregnancy with HBeAg(-)(P<0.01). Meantime, in the groups with mother pregnancywithHBeAg(+) or HBV DNA(+), the rate of passing42-days hearing screening wassignificantly lower than the rate in the groups of mother pregnancy with HBeAg(-) or HBV DNA(-)(P<0.05). Conclusion There was interactive relationships between mother pregnancy with HBeAg(+) or HBV DNA(+) and neonates intrauterine infection as well as failed in hearing screening in our research. In the future research, we should pay more attention and follow-up regularly.
