目的:探讨MRI动态增强血管成像在乳腺良恶性病变诊断中价值,提高正确诊断率。方法:收集符合标准的患者60例,分析MRI动态增强资料。结果:60例患者病理结果显示乳腺恶性肿瘤37例、乳腺良性病变23例;MRI动态增强诊断正确51例,敏感性86.84%,特异性81.82%。乳腺恶性病变时间-信号强度曲线以Ⅲ型为主(70.27%),乳腺良性病变以Ⅰ型为主(73.91%),差异有统计学意义(P<0.05)。乳腺恶性病变早期强化率和峰值强化率均高于乳腺良性病变,差异有统计学意义(P<0.05)。结论:MRI 动态增强血管成像能显示乳腺肿块的形态学和血流动力学特征,具有较高的敏感性及特异性,为乳腺病变定性诊断及治疗提供重要的依据。

Objective:To investigate the values of dynamic contrast enhanced MRI in diagnosis of dis-eases of mammary gland and to elevate the correct diagnosis percentage .Method:60 eligible patients were enrolled and the dynamic contrast enhanced MRI data was analyzed .Result:As diagnosed by the pathologic results,37 of the 60 patients had malignant tumors of mammary gland , and 23 cases had benign lesions of mammary gland;51 cases were correctly diagnosed by dynamic contrast enhanced MRI , and the sensitivity and specificity were 86.84%and 81.82%respectively.The time-signal intensity curves of malignant tumors of mammary gland were mainly type III (70.27%), while those of benign lesions of mammary gland were mainly type Ⅰ(73.91%), and the difference was statistically significant (P<0.05).The early enhancement rate and peak enhancement rate of malignant tumors of mammary gland were both higher than those of benign lesions of mammary gland , and the differences were statistically significant ( P<0.05) .Conclusion:Dynamic contrast enhanced MRI can manifest the morphologic and hemodynamic characteristics of breast lump , has comparatively high sensitivity and specificity , and can provide important foundations for qualitative diagnosis and therapy of diseases of mammary gland .
