转变“以药补医”模式、完善补偿机制是当前医改难度最高、操作最复杂的领域。上海市公立医院“医药分开”改革的基本思路是通过取消药品加成，合理调整医务人员劳务价格，优化药品供应链降低运行成本，适度增加医疗保障基金支出和财政补贴等措施，使居民就医费用总体有所下降，公立医院收入总体平衡、结构明显优化，减少对“大处方”的趋利行为。

It is the most difficult and challenging task to change the mode of ‘Cover Hospital Expense with Medicine Revenue’ to that of improving the compensation mechanism in current health care system reform. The main thoughts in the reform of‘Separating Pharmacies from Medical Institutions’ in Shanghai public hospitals take the methods of cancellation the profit of drugs, rationally adjusting the manpower price, optimizing drug supply chains, reducing operation cost, and properly increasing health insurance fund expenditure and financial subsidies. As a result, the cost of out-of-pocket is reduced; the income and cost of public hospital is kept balanced and significantly optimized, and prevented the trend of making profit from excessive prescription.
