目的：评价纤支镜肺泡灌洗并注药治疗难治性肺炎临床应用价值。方法：将笔者所在医院收治的难治性肺炎的38例患者，随机分为对照组及治疗组。对照组采用常规治疗，治疗组除常规治疗外，反复经纤支镜肺泡灌洗注药，比较两组治疗效果。结果：22例治疗组患者中，显效11例，好转6例，无效5例；16例对照组患者中，显效5例，好转4例，无效7例。与对照组相比，治疗组的总效率明显高(P<0.05)。另外，治疗组住院天数、治疗费用明显好于对照组；在纤支镜灌洗治疗过程中，无严重并发症出现。结论：纤支镜肺泡灌洗并注药治疗难治性肺炎能够提供更多的细菌学依据，减少患者住院费用，缩短住院天数，是一种有效的治疗方法，有重要临床应用价值。
목적：평개섬지경폐포관세병주약치료난치성폐염림상응용개치。방법：장필자소재의원수치적난치성폐염적38례환자，수궤분위대조조급치료조。대조조채용상규치료，치료조제상규치료외，반복경섬지경폐포관세주약，비교량조치료효과。결과：22례치료조환자중，현효11례，호전6례，무효5례；16례대조조환자중，현효5례，호전4례，무효7례。여대조조상비，치료조적총효솔명현고(P<0.05)。령외，치료조주원천수、치료비용명현호우대조조；재섬지경관세치료과정중，무엄중병발증출현。결론：섬지경폐포관세병주약치료난치성폐염능구제공경다적세균학의거，감소환자주원비용，축단주원천수，시일충유효적치료방법，유중요림상응용개치。
Objective:To evaluate the value of clinical application of the fiberoptic bronchoscopy alveolar lavage and injecting drugs in the treatment of refractory pneumonia through.Method:38 cases of refractory pneumonia patients in our hospital were randomly divided into the control group and treatment group. The control group with conventional therapy,the treatment group in addition to conventional treatment,through fiberoptic bronchoscopy alveolar lavage and injecting drugs over and over again,two groups of therapeutic effect was compared.Result:22 cases in treatment group patients,11 cases had significant effect, 6 cases got better,5 cases ineffective;16 cases in the control group patients,5 cases had significant effect,4 cases got better,7 cases ineffective.Compared with the control group,the total efficiency of treatment group was obviously higher(P<0.05).In addition,the treatment group were much better than the control group in hospitalization days and cost of treatment,and in the process of fiber bronchoscope lavage treatment,no serious complications.Conclusion:The treatment of refractory pneumonia through Fiberoptic bronchoscopy alveolar lavage and injecting drugs can provide more basis of bacteriology reduce the in-hospital costs, shorten the hospitalization days,is an effective method of treatment,has important clinical value.
