目的:探讨血清胱抑素C( Cys C)对造影剂肾损伤早期诊断的价值。方法:选择140肾脏造影患者,造影前、造影后12、24、48 h分别检测血清Cys C、血肌酐( Scr)、血尿素氮( BUN)、尿酸( UA)并推算出肌酐清除率( CCr)。结果:与造影前相比:造影后12、24、48 h Cys C浓度有明显升高,且差异有统计学意义( P均<0.05);造影后12、24、48 h Scr、BUN、UA变化均差异无统计学意义( P均>0.05)。造影后24 h Cys C受试者工作特征曲线( ROC)下面积( AUC)最大为0.88,诊断效能较高。结论:血清Cys C是反映造影剂早期肾损伤的敏感指标。

Objective To explore the value of serum cystatin C( Cys C)in early diagnostic contrast induced renal injury. Method The total 140 patients,who had been successfully carried on kidney angiography. Cys C,serum creatinine( Scr),blood urea nitrogen( BUN),u-ric acid(UA)were detected before and after 12,24,48 hours,and the creatinine clearance rate(CCr)was calculated. Results Cys C was statistically higher at 12,24 and 48 hours after kidney angiography,and the differences were significant(all P<0. 05). The differences of Scr,BUN,UA between and after kidney angiography were not significant( all P>0. 05). The area under the curve( AUC)under the receive operating characteristic curve( ROC)was 0. 88 and the diagnostic efficiency was the highest when the Cys C was 24 hours after kidney angi-ography. Conclusion Serum cystatin C is a sensitive biomarker to reflect early contrast renal injury.
