目的：探讨传统三孔、单操作孔及单孔VATS（Video-assisted thoracoscopic surgery，VATS）在治疗年轻患者自发性气胸治疗中临床效果。方法：自2011年4月-2013年10月期间，采用胸腔镜治疗年轻患者自发性气胸48例。依照住院次序分别行三孔VAT S法16例，单操作孔VAT S 法23例，单孔VAT S法9例，对3组患者相关临床指标进行分析比较。结果：3组患者在手术时间、术中出血量、术后总引流量、拔管时间、手术相关并发症、住院时间比较差异无统计学意义（P>0.05）。结论：单操作孔及单孔VAT S在治疗年轻患者自发性气胸方面与传统三孔式VAT S相比，疗效相似，而手术创伤小，患者的满意度高，值得临床推广。

Objective:To discuss the clinical effect of standard three-port, one single utility port and uniportal VATS on the treatment of spontaneous pneumothorax in young patients.Methods:From April 2011 to October 2013, 48 young patients with spontaneous pneumothorax performed operation in VATS. According to hospital order, group A(16 cases) underwent approach with standard three-port VATS, Group B(23 cases) with one single utility port VATS, and group C( 9 cases) with uniportal VATS .Clinical data of the 3 groups were analyzed.Results:There were no statistical signiifcance between 3 groups in the operative time, blood loss, postoperative total drainage, tube removed time, complications related to surgery, and hospital stay(P> 0.05).Conclusions:Compared to standard three-port VATS , one single utility port and uniportal VATS for spontaneous pneumothorax in young patients have the same therapy effect, smaller surgical trauma, and higher satisfaction . In clinical practice, they deserve to be used widely.
